吉西他滨联合奥沙利铂治疗老年晚期食管癌的临床观察

目的 观察吉西他滨联合奥沙利铂(GEM0X)方案治疗晚期老年食管癌的疗效和毒副反应.方法 经病理或细胞学证实的64例晚期老年食管鳞癌患者随机分成两组,观察组34例,采用GEMOX方案,对照组30例,采用顺铂(DDP)联合氟尿嘧啶(5-FU)组成PF方案治疗,分别观察两组疗效及毒副反应.结果 观察组及对照组有效率分别为41.2％和40.0％,两组差异无统计学意义(P＞0.05)；毒副反应主要为骨髓抑制、肾功能损害及恶心等消化道反应,其中观察组Ⅲ/Ⅳ级白细胞下降、Ⅲ/Ⅳ级恶心呕吐等消化道反应及Ⅰ/Ⅱ级肾功能损害明显轻于对照组(P＜0.05).结论 吉西他滨联合奥沙利铂治疗老年晚期食管癌的疗效与顺铂联合氟尿嘧啶组成的PF方案相比虽未有提高,但毒副反应较轻,患者易耐受.

Effect of gemcitabine combined with oxaliplatin in the treatment of elderly patients with advanced esophageal cancer

Objective To observe the effect of gemcitabine combined with oxaliplatin for treating elderly patients with advanced esophageal cancer.Methods A total of 64 patients confirmed as esophageal cancer through pathology or cytology were randomized into two groups:the study group(n=34) and the control group(n=30).Patients in the study group were treated with GEMOX regimen(gemcitabine plus oxaliplatin),while those in the control group were treated with PF regimen(Fluorouracil plus Cisplatin).The efficacy and adverse reactions were compared between the two groups.Results The overall response rate was 41.2% in the study group and 40.0% in the control group,showing no statistically significant difference between the two groups(P>0.05).The most common adverse reactions were hematologic suppression,disgusting,as well as renal and neurologic damage.The incidence of grade Ⅲ/Ⅳ leukopenia,Ⅲ/Ⅳ alimentary and Ⅰ/Ⅱrenal damage in the study group were significantly lower than those in the control group(P<0.05).Conclusion Although the efficacy of GEMOX regimen is similar to that of PF regimen in the treatment of elderly patients with advanced esophageal cancer,the adverse reaction of GEMOX regimen less serious.
