输卵管妊娠腹腔镜保守治疗与药物治疗后妊娠相关因素分析

目的 比较腹腔镜保守手术治疗与化学药物保守治疗输卵管妊娠的效果.方法 100例输卵管妊娠患者分为腹腔镜保守手术治疗组(A组50例)和化学药物保守治疗组(B组50例),比较两组患侧输卵管通畅率、宫内妊娠率及患侧输卵管再次异位妊娠率等.结果 A组患侧输卵管通畅率76％,宫内妊娠率70％,患侧输卵管再次异位妊娠12％;B组患侧输卵管通畅率40％,宫内妊娠率48％,患侧输卵管再次异位妊娠28％.两组比较差异有统计学意义(P＜0.05).结论 输卵管妊娠保守治疗后输卵管的功能取决于其通畅情况,通过宫腔镜输卵管插管通液可有效地判断输卵管通畅情况；就提高今后宫内妊娠率、减少患侧输卵管再次异位妊娠率等方面,腹腔镜保守手术均优于药物治疗.

Analysis on the pregnancy-related factors after laparoscopic oviduct-sparing operation and drug therapy for tubal pregnancy

Objective To compare the effect of laparoscopic oviduct-sparing operation and drug therapy in the treatment of tubal pregnancy.Methods 100 patients with tubal pregnancy were divided into two group.Patients in group A(n=50) were treated with laparoscopic oviduct-sparing operation,while those in group B(n=50) were treated with drug therapy.The tubal patency rate,intrauterine pregnancy rate and the recurrent tubal pregnancy rate were compared between the two groups.Results The tubal patency rate,the intrauterine pregnancy rate,and the recurrent tubal pregnancy rate was 76%,70%,12% in group A and 40%,48%,28% in group B,respectively,showing statistically significant difference between the two groups.Conclusion The function of fallopian tube after conservative management of tubal pregnancy depends on tubal patent status.Hysteroscopic tubal catheterization liquid instillation treatment can judge the patent status for the tubes effectively.The laparoscopic oviduct-sparing operation is better than the drug therapy in the treatment of tubal pregnancy for improving intrauterine pregnancy rate and reducing pregnancy tubal rate.
