腹膜透析联合短时血液滤过治疗急性重症胰腺炎的临床观察

目的探讨腹膜透析联合短时血液滤过治疗急性重症胰腺炎的治疗作用。方法将36例急性重症胰腺炎患者作为治疗组,与对照组62例进行对照,比较两组的腹痛、腹胀缓解时间、住院天数、住院费用、转手术率、器官衰竭发生率和治愈率。治疗组在常规保守治疗基础上使用腹膜透析联合血液滤过,对照组常规保守治疗。结果治疗组的腹痛、腹胀缓解时间、住院天数、住院费用、转手术率、器官衰竭发生率均较对照组显著降低(P<0.01),治愈率较对照组提高(P<0.05)。结论运用腹膜透析联合短时血液滤过治疗急性重症胰腺炎效果明显,对于缓解症状、缩短疗程、防止或减少器官衰竭发生及改善愈后有显著作用,早期运用效果明显。

Clinical observation of short-time hemofiltration and peritoneal dialysis on severe acute pancreatitis

Objective To study the therapeutic effects of combination of short-time hemofiltration(HF) and peritoneal dialysis (PD) on severe acute pancreatitis(SAP).Methods 98 patients with SAP were divided randomly into the therapeutic group (36 patients) and the contrast group (62 patients) . Patients in the therapeutic group were treated with PD and short-time HF besides normal conservative therapy. And patients in the contrast group were treated with normal conservative therapy. The abdominal pain, the time to release the puffy abdomen, the time of hospitalization ,cost of treatment in hospital , operative rate and rate of organ failure of the two groups were compared.Results The abodominal pain, the time to release the puffy abdomen, the time of hospitalization,cost of treatment in hospital , operative rate and rate of organ failure of the therapeutic group were significantly decreased compared with those of the contrast group. Healing rate of the therapeutic group was raise compared with those of the contrast group. Conclusion The effect of combination of short-time hemofiltration and peritoneal dialysis on remising syndrome, shortening course of treatment, preventing or reducing the occurrence of organ failure and improving prognosis of patients with severe acute pancreatitis(SAP) was significant. The earlier the treatment of HF + PD, the better the effect. 【
