外周血CD64表达对鉴别诊断小儿急性上呼吸道感染的价值

摘    要:目的应用流式细胞仪测定小儿急性上呼吸道感染患儿外周血中性粒细胞CD64表达水平.用于鉴别诊断细菌性感染和病毒性感染。方法随机抽取59例本院住院小儿急性上呼吸道感染患儿,根据白细胞计数,中性粒细胞分类及病原学和血清学检查等分为两组,其中细菌性上感31例,病毒性上感28例,测定CRP及CD64水平,并与正常对照组比较:同时根据白细胞数量分为白细胞高值组和白细胞低值组,观察两组之间的CD64水平。结果31例细菌性上感CD64表达量明显高于正常对照组(P〈0.01),CRP阳性27例,阴性4例.阳性率88%,而28例病毒性上感CD64表达量与正常对照组比较差异无显著性(P〉0.05);CRP阳性6例,阴性22例,阳性率22%:同时白细胞高值组CD64表达量明显高于正常对照组(P〈0.01),而白细胞低值组与正常对照组比较差异无显著性(P〉0.05)。结论小儿急性上呼吸道感染患儿外周血中性粒细胞CD64表达水平与CRP、白细胞数量呈正相关,测定外周血CD64可用于鉴别细菌性感染和病毒性感染。

Abstract:Objecive To approach into the expression of peripheral blood neutrophil CD64 in children with acute upper respiratory tract infection for authenticating bacterial orand virus infection. Methods Peripheral blood neutrophil CD64 was detected by flow cytometryin 59 children with acute upper respiratory tract infection (31 cases of bacterial infection and 28 cases of virus infection),CRP was detected by immunoturbidimetric method. Results CD64 expres- sion in bacterial infection group was higher than thatin controls. Ther ewere27 cases of CRP positive and 4 cases of CRP negative in bacterial infectiongroup. CD64 expression was nosig- nificantly defferent between virus infection and control group. CRP showed 6 cases positive and 22cases CRP negative in virus infection group. CD64 expression in the cases with leucocytosis was higher than that in leucopenia. Conclusions It exists a positive correlation between CD64 expression and CRP level or the quantily of white blood cell. Peripheral blood CD64 detection can be used to authenticate bacterial or virus infection.
