提上睑肌折叠术矫治上睑下垂的临床观察

目的探讨提上睑肌折叠术治疗上睑下垂的临床疗效。方法选择216例(273只眼)上睑下垂患者,采用提上睑肌折叠术。结果术后随访6个月~4年,216例(273只眼)正矫241只眼(88.28%),欠矫29只眼(10.62%),过矫3只眼(1.10%)。其中提上睑肌肌力良好者101只眼,正矫91只眼(90.09%),欠矫9只眼(8.91%),过矫1只眼(0.99%);提上睑肌肌力中等者126只眼,正矫110只眼(87.30%),欠矫14只眼(11.11%),过矫2只眼(0.79%);提上睑肌肌力弱者46只眼,正矫40只眼(86.96%),欠矫6只眼(13.04%),无一例过矫。统计学分析提上睑肌肌力良好、中等、弱者三者之间术后矫治效果无显著性差异。结论提上睑肌折叠术符合生理解剖特点,手术简化,术后反应轻恢复快,效果可靠满意,是矫治上睑下垂的一种理想手术方法。

Levator muscle pverlapping technique for the correction of ptosis

Objective To investigate the effect of levator muscle overlapping technique on ptosis.Methods 216 cases (273 eyes) with ptosis received levator muscle overlapping technique.Results All patients were followed up from 6 months to 4 years.In these 273 eyes,241 eyes (88.28%) were correctly positioned ,29 eyes (10.62%) were under-corrected,3 eyes (1.10%) were over correction.Among 101 eyes with levator functions was more than 8mm,91 eyes (90.09%) were correctly positioned,9 eyes (8.91%) were under-corrected,and 1 eye (0.99%) was over-corrected; Among 126 eyes with levator functions was 4~7mm, 110 eyes (87.30%) were correctly positioned,14 eyes (11.11%) were under-corrected,and 2 eyes (0.79%) were over-corrected; Among 46 eyes with levator functions was 0~3mm,40 eyes (86.96%) were correctly positioned,6 eyes (13.04%) were under-corrected,and none was over-corrected.There was no statistically difference in post surgical result between levator functions at more than 8mm,4~7mm,and 0~3mm.Conclusion It shows that the levator muscle overlapping technique is an ideal method to treat the patients with ptosis.the advantages include that the surgery is simple,recovery period is shortened,the corrected position is satisfied,and the procedures are fitted well with patient's anatomy.
