肺吸虫病结核病双联快速诊断方法的建立

摘    要:目的创建具有肺吸虫病、结核病诊断和鉴别诊断价值的血清抗体检测新技术。方法以硝酸纤维素膜为载体,在膜上左右对称点样肺吸虫成虫可溶性抗原、结核分支杆菌脂阿拉伯甘露糖(LAM)抗原,以胶体金-羊抗人IgG结合物为标记物,创建肺吸虫病、结核病双联金标免疫快速诊断方法(Pw/TB-DIGFA),检测病人血清样本中抗肺吸虫和/或抗结核杆菌IgG抗体。结果Pw/TB-DIGFA诊断肺吸虫病和结核病的敏感性分别为96.7%(29/30)和91.1%(51/56),特异性为92.6%(100/108);与TB-Dot平行检测结核病人和健康人群血清94人份,结果差异无统计学意义,符合率达96.8%(91/94);与Pw-DIGFA平行检测肺吸虫病人和健康人群血清90人份,检测结果一致;未发现相互之间有交叉反应。结论Pw/TB-DIGFA对肺吸虫病和结核病诊断具有较高的敏感性和特异性;该方法一次操作同时完成二项特异性抗体测定,不仅检测效率高而且平行检测结果有利于鉴别诊断。

Abstract:Objective To develop a new immunoassay for co-diagnosis both paragonimiasis and tuberculosis.Methods The nitrocellulose(NC)membrane was used as solid vector in the new immunoassay.Adult soluble antigen of Paragonimus westermani and Lipoarabinomannan(LAM)of Mycobacterium tuberculosis were dotted on the NC membrane separately.Antibody of goat anti-human IgG was conjugated with colloid gold as the detecting marker.Double dot immunogold filtration assay for paragonimiasis and tuberculosis(Pw/TB-DIGFA)was designed to detect the specific IgG to P.westermani and/or LAM of M.tuberculosis in patient's sera.Results Sensitivities of this new immunoassay were 96.7%(29/30)and 91.1%(51/56)for paragonimiasis and tuberculosis respectively.The specificity was 92.6%(100/108).Ninety-four sera were detected in parallel with Pw/TB-DIGFA and TB-Dot,and there was not significant difference.The result of 90 serum detected by Pw/TB-DIGFA agreed to that by Pw-DIGFA.Cross reaction between P.westermani and LAM of M.tuberculosis was not found in this study.Conclusion Pw/TB-DIGFA as a new immunoassay was sensitive and specific for diagnosis for paragonimiasis and/or tuberculosis.This immunoassay was not only more effective but also favourable to analyze the results when it could fulfill two specific antibody detection against P.westermani and P.tuberculosis simultaneously.
