肾上腺皮质癌的诊断与治疗(附19例报告)

目的：探讨肾上腺皮质癌的诊断与治疗方法。方法：回顾性分析了19例肾上腺皮质癌患者的临床资料，本组19例，术前作B超(19例)、CT(17例)、IVP(6例)检查。均经手术治疗并经病理证实。结果：本组术前16例确诊，3例未确诊。其中B超及CT的确诊率分别为68．4％(13／19)、76.5％(13／17)。本组术后平均生存期31．3个月，3年生存率为15．8％(3／19)，5年生存率为11．8％(2／17)。结论：肾上腺皮质癌恶性程度高，影像学检查是确诊的关键；早期发现并行根治性切除手术是目前提高其生存率的唯一有效方法。

Diagnosis and Management of Adrenocortical Carcinoma

Objective:To discuss the diagnosis and treatment of adrenocortical cinomas. Methods:19 cases of adrenocortical carcinoma were reviewed,B-US(19cases),CT(17cases)and IVP(6cases)estimations had been conducted before operation,All cases were treated by surgery and proven by pathology. Result:Accuracy rate of diagnosis by BUS and CT was 68.4%(13/19)and 76.5%(13/17),respectively. The mean survival duration was 31.3 months,The 3-year survival rate was 15.8%(3/19),5-year survival rate was 11.8%(2/17). Conclusion:Adrenocortical carcinoma is a lethal malignancy,Radiologic evaluation is the key way.Earlier detection and complete initial surgical resection may be the best treatment at present.
