烧伤早期血小板计数的临床意义

摘    要:目的探讨烧伤早期血小板变化与病情及转归的关系。方法对120例烧伤病人(总烧伤面积〉30%TBSA者44倒,〈30%TBSA者76例)入院早期血小板水平进行测定,并结合临床病情及转妇进行时比分析。结果总体上看.烧伤早期,血小板变化呈现出短暂升高、持续下降、然后恢复正常的趋势。总烧伤面积〉30%TBSA纽血小板值呈下降趋势(P〈0.05),〈30%TBSA组血小板值正常(P〈0.05);血小板水平与TBSA显著相关(P〈0.01);血小板≤60×10^9/L的痛人死亡率显著增高(P〈0.01),以血小板≤60×10^9/L作为预后判断标准,灵敏度为80%,特异性为99.1%。结论烧伤早期血小板变化与烧伤程度有关,并可预示其转妇,治疗中注意及早进行血小板干预可能有助于病人的救治。

The Clinical Significance of Platelet Count in Early Stage of Burn

Abstract:Objeetive To explore the change of platelet count in early stage of burn and the relation.ship between the .severity and progno-sis. Methods 120 eases of burn patients' (total burn surface area 〉 30% TBSA in 44 cases, 30% TBSA in 76 cases) platelet levels were deter-mined in early hospital admission, and the combination tff clinical severity and prognosis were analyzed. Results Look on the whole, in early stage of burn, platelet count change showed the trend of a transient rising, continued to decline, then returned to the normal level. The total burn surface area 〉 30 % TBSA group platelet values decreased (P 〈 0.05), 〈 30% TBSA group platelet values remained normal (P 〈 0.05) ; platelet levels were significantly as.sociated with TBSA ( P 〈 0.01 ) ; platelet ≤ 60×10^9/L patient mortaliLx rates increased significantly ( P 〈 0. 01 ), platelet ≤60×10^9/L as a prognostic criterion, sensitivity was 80%, specificity was 99.1%. Conclusions Platelet levels in early stage of burn wear related to the degree of burns, and may indicate its prognosis, treatment of attention to early platelet intervention may contribute to patient treatment.
