外周血Tg mRNA的检测及诊断甲状腺癌的临床意义

摘    要:目的:探讨检测外周血中甲状腺球蛋白mRNA对甲状腺癌的诊断价值。方法:应用逆转录聚合酶链反应(RT-PCR)技术,对55例患者(39例甲状腺癌,7例甲状腺良性肿瘤,9例非甲状腺肿瘤)进行循环血中甲状腺球蛋白mRNA检测,39例甲状腺癌中34例乳头状癌,2例滤泡癌,3例髓样癌。结果:39例中12例发现Tg mRNA(31%),其中11例为乳头状癌,1例滤泡癌,7例甲状良性肿瘤和9例非甲状腺肿瘤未发现Tg mRNA 。40%颈淋巴结转移和100%远处转移的病例Tg mRNA阳性,而无转移的病例无论原发肿瘤切除与否Tg mRNA均阴性。结论:外周血中Tg mRNA可作为甲状腺癌的肿瘤标志物,特别对颈部或远处转移具有一定的临床诊断价值。

Abstract:Objective:To study the diagnostic value of circulating thyroglobulin(Tg)mes-senger ribonucleic acid(mRNA)in peripheral blood in patients with thyroid carcinoma.Meth-ods:The expression of Tg mRNA was determined in peripheral blood of55patients including thyroid cancer39(35papillary,2follicular and3medullary),benign thyroid disease7and non-thyroid tumor patients9using RT-PCR assay.Results:Of 39thyroid cancer patients,Tg mR-NA was positive in12cases(31%)including papillary carcinoma 11,follicular1.Tg mRNA was positive in40%of patients with cervical metastases and100%with distant metastases.Tg mRNA was not detected in any patients without metastasis whether the tumor of thyroid was re-sected or not.Conclusion:We have demonstrated that Tg mRNA can be detected in peripheral blood of thyroid carcinoma patients especially in patients with cervical or distant metastases.
