罗哌卡因复合芬太尼在无痛分娩中对母婴的影响

目的观察硬膜外联合应用罗哌卡因及芬太尼在无痛分娩中对母婴的影响。方法随机根据所注射的成份将所有进行分娩的患者分为三组：A组单纯用0.2%罗哌卡因,以10 ml/h行硬膜外自控镇痛（PCEA）;B组用0.1%罗哌卡因混合1.5μg/ml芬太尼,以10 ml/h行PCEA;C组未进行无痛分娩,每组各64例。对各组患者视觉模拟镇痛情况、脐血血气分析、总产程时间、分娩方式和宫缩素使用、新生儿Apgar评分以及整个分娩期间产妇血压、心率情况进行统计分析对比。结果 A组和B组相对于未进行无痛分娩的C组,产妇由疼痛引起的应激反应明显减轻;A组和B组的剖宫产率要显著低于C组;而A组的器械助产率均高于B组和C组;三组脐带血气分析差异不明显,其结果无统计学意义。结论罗哌卡因联合芬太尼用于无痛分娩对于产妇疼痛缓解明显,可降低剖宫产率,而不增加器械助产率,对母婴无不良影响,值得临床推广。

Ropivacaine combined with fentanyl in painless childbirth during the maternal

Objective To observe the effect of epidural ropivacaine combine with fentanyl in the painless childbirth of maternal and child.Methods According to the composition of drugs,all patients were divided into 3 groups: A group with managed with 0.2% ropivacaine;B group with 0.1% ropivacaine mixed 2.5μg/ml fentanyl,C group with no drugs,with 64 cases in each group.Visual analogue pain conditions,cord blood gas analysis,the total production process,delivery methods and uterine hormone,the apgar score,the maternal blood pressure and heart rate,during childbirth,were record and compared between two groups.Results Compare with that in group C,maternal stress caused by the pain in group A and group B reduced significantly.The cesarean section rate in group A and group B was significantly lower than that in group C,and devices help rate in group A was higher than that in group B and group C,and there was no significant difference between three groups in umbilical cord blood gas analysis.Conclusion Combination of ropivacaine and fentanyl can relief painful during the maternal,with no side effects.
