ERCP后CT扫描对导致阻塞性黄疸的胰胆疾病诊断价值的研究

目的　联合应用ERCP和ERCP后CT扫描 ,以提高对导致阻塞性黄疸的胰胆疾病诊断的准确性。方法　前瞻性地对5 5例阻塞性黄疸ERCP检查不能确定的病例 ,于拔管后 2小时内立即行CT扫描。由三位专科医生按照统一观察指标进行评价。结果　 5 5例中 2 4例为胰胆恶性肿瘤 ,3 1例为良性病变。ERCP诊断完全正确率 5 8 2 % (3 3 / 5 5 ) ,ERCP后CT扫描为 92 7% (5 1/ 5 5 ) ,P<0 0 5。统计学处理有显著性差异。结论　ERCP后CT扫描是一种有效的检查方法 ,可提高病因诊断正确率。

Evaluating study of CT scanning after ERCP for the clinical diagnosis of pancreaticobiliary diseases which cause obstructive jaundice

Aim To increase the reconfirm of jaundice induced by pancreaticobiliary diseases by means of combining ERCP with CT scanning after ERCP.Methods Fifty five patients had been examined by the designed program.First, ERCP was taken, then followed CT scanning immediately after ERCP, because some features of ERCP were uncertain.According to the same criteria, evaluation of etiologic diagnosis was given by 3 doctors.Results Twenty four and thirty one cases were caused by malignant and benign diseases,respectively. The rate of correct diagnosis of ERCP was 58.2% (32/55), and that of CT scanning after ERCP was 92.7%(51/55).( P <0 05).The results had significant difference. Conclusions CT scanning after ERCP is an effective complemental examination for ERCP. The combined method can increase the diagnosis accuracy of etiology of jaundice induced by pancreaticobiliary diseases.
