速度向量成像技术在肥厚型心肌病及继发性心肌肥厚患者左室扭转分析中的应用

摘    要:目的探讨速度向量成像(velocity vector imaging,VVI)技术对肥厚型心肌病及继发性心肌肥厚患者左室扭转运动特征的分析。方法根据超声心动图等检查结果,将47例受检者分为正常组(20例)、肥厚型心肌病组(12例)和继发性心肌肥厚组(15例),采用Siemens公司Sequoia512超声诊断仪,在VVI模式下对受检者取心底部短轴切面和心尖部短轴切面,勾画心内膜,应用VVI软件自动跟踪分析左室扭转角度。结果左室扭转角度,正常组为9.84°±1.99°,肥厚型心肌病组为6.49°±2.66°,继发性心肌肥厚组为7.40°±2.46°。正常组与肥厚型心肌病组及继发性心肌肥厚组比较,差异均有统计学意义(均P〈0.05)。肥厚型心肌病组与继发性心肌肥厚组比较,差异无统计学意义(P〉0.05)。结论VVI技术是一种方便、无创的测量心脏扭转的方法。

Abstract:Objective To observe the difference of left ventricular twist between hypertrophic cardiomyopathy and secondary cardiomyopathy using velocity vector imaging (VVI). Methods Images of the left ventricle were acquired at the apical and mitral short-axis levels in normal group, hypertrophic cardiomyopathy group and secondary cardiomyopathy group. The left ventrieular twist was examined with VVI software. Results The left ventricular twist in normal group was 9.84°±1.99°. The left ventricular twist in patients with hypertrophic cardiomyopathy and secondary myocardial hypertrophy were 6.49°±2.66° and 7.40°±2.46°, respectively. The difference in left ventricular twist between normal group and the two abnormal groups were both significant (all P〈 0. 05 ). The difference in left ventricular twist between the two abnormal groups was nonsignificant (P〉 0.05). Conclusion Left ventricular twist can be measured noninvasively by VVI with high feasibility. This novel method allows the detailed study of cardiac twist in patients with myocardial hypertrophy.
