老年股骨粗隆间骨折外固定术

探讨老年股骨粗隆间骨折外固定术的可行性。方法闭合复位，模拟股骨粗隆部骨小梁的排列走向，于非同一平面向股骨头颈内交叉穿入三枚斯氏针，股骨上段穿入二枚螺纹半针形成不规则四边形固定。结果无脱针，断针，头切割，无髋内翻，无再手术，无手术死亡。

External Fixation in the Treatment of Senile Intertrochanteric Fractures

Objective To study the possibility of external fixation in the treatment of senile intertrochanteric fracture.Methods After closing reduction,three Steinmann's pins were crosswise inserted into the head and neck of femur on different planes along the direction of trochanteric trabecularism and two screw pins were inserted into the upper femur,constructing an irregular quadrilateral frame.Results The excellent good rate for 38 followed up cases was 94.7%.None of the following events was happened,i.e.broken pin,falling off pin,head cut,coxa vara,reoperation,and operative death.The track reaction rate was high,but the track infection rate was low.Conclusion External fixation causes minor trauma and less bleeding,takes shorter time of operation,and does not require transfusion.Therefore,it ensures the security in the treatment of intertrochanteric fractures in the aged and weak,as well as those who have less endurence to surgical operation.
