他汀类药物与其他药物的相互作用

摘    要:3-羟-3-甲基戊二酰辅酶A(HMG-CoA)还原酶抑制剂(他汀类)之间存在药动学差异,使它们与其他药物发生相互作用的机制及程度不同。主要经细胞色素P450(CYP)3A4酶代谢的洛伐他汀、辛伐他汀、阿托伐他汀、西立伐他汀的药物相互作用多见,其中,西立伐他汀因其致命性横纹肌溶解已被迫退出世界市场。经CYP2C9酶代谢的氟伐他汀、罗舒伐他汀与其他药物联合应用时,肌病发生率很低。而不通过CYP系统代谢的普伐他汀耐受性好,肌病及肝功能异常者罕见。

Abstract:Different pharmacokinetics exist among HMG-CoA reductase inhibitors (statins) proroking various mechanisms and extents of drug interaction. Lovastatin, simvastatin, atorvastatin and cerivastatin are mainly metabolized via the cytochrome P450(CYP)3A4 pathway with common occurrence of drug interactions. The worldwide withdrawal of cerivastatin in August 2001 because of its association with fatal rhabdomyolysis. Fluvastatin and rosuvastatin metabolised by the cytochrome CYP2C9 exhibit very low propensities to elicit myopathy combining with other agents. Pravastatin does not metabolize via the cytochrome P450 system,and also well possesses tolerance with rare incidences of myopathy and abnormal liver function test.
