中医气郁体质与脑卒中后抑郁症证候关系研究

目的：从中医体质学角度探讨脑卒中后抑郁症（PSD）证候的相关性？方法：选择符合PSD诊断标准的106例患者，以中医9种体质分型为标准，选出具有中医气郁体质的PSD组30例，瘀血体质组30例，和平体质组30例。分析对比3组的汉密尔顿抑郁量表评分（HAMD），中国临床神经功能缺损程度评分（SSS），改良巴氏指数评分（ADL）以及中国抑郁症证候分类等指标。结果：中医气郁体质PSD患者HAMD、SSS、ADL的不同程度评分均数与瘀血体质组比较有差异，与和平体质患者比较有显著差异，抑郁程度高于其他两组。结论：气郁体质PSD患者有较高抑郁倾向，神经功能缺损严重，恢复时间长。并具有易激怒和怯懦易悲的临床表现。

Syndrome Relationship Between TCM Qi-stagnation Constitution and Post-stroke Depression

To probe the related syndromes of post-stroke depression （PSD） from the perspective of TCM physical science. Methods： According to nine kinds of TCM constitution standard, 30 patients with TCM qi-stagnation constitution, 30 patients with blood stasis, 30 patients with peace constitution were selected from 106 patients conforming to PSD diagnosis standard. Then HAMD, SSS, ADL and Chinese depression syndromes of 3 groups were analyzed and compared. Results ： The average scores of HAMD, SSS, ADL of different levels of PSD patients with TCM qi-stagnation constitution were different from those of the blood stasis group, and comparing with the patients with peace constitution, there was significant difference, and the degree of depression was higher than that of other two groups. Conclusion： PSD patients with qi-stagnation constitution have a higher depression tendency, severe neurologic impairment, with a long recovery time, And they have the clinical manifestations of easy anger and cowardice and easy grief.
