种植体周围龈沟液量的测定及意义

目的通过检测种植体周围不同健康状态的龈沟液量,初步探讨种植体周围龈沟液量与种植体周围组织健康状态的关系。方法将29例接受种植义齿治疗患者的40枚种植体及口内同名健康牙,分为种植体组和正常对照组,种植体组又按临床及影像学标准划分为炎症组和健康组,其中炎症组25枚种植体,健康组15枚种植体,记录受检种植体和健康牙的各项临床指标,采用滤纸条法收集龈沟液,测定龈沟液量。结果①正常对照组、健康种植体组和炎症种植体组之间的龈沟液量、菌斑指数(PLI)、改良龈沟出血指数(MBI)及探诊深度(PD)均有非常显著性差异(P<0.001),其中炎症种植体组的龈沟液量和各项临床指标均大于正常对照组和健康种植体组,有显著性差异(P<0.05),正常对照组和健康种植体组之间的龈沟液量和各项临床指标无显著性差异(P<0.05)。②种植体组龈沟液量同各项临床指标之间都有显著的正相关关系,种植体组龈沟液量与PLI、MBI、PD值的相关系数r和P值分别为0.631,0.001;0.502,0.012;0.546,0.006。结论龈沟液量可以在一定程度上反映种植体周围组织的健康状态,有可能作为评价种植体周围组织健康状态的指标。

Assay and significance of peri-implant sulcular fluid volume

Objective To investigate the volume of peri-implant sulcular fluid(PISF) for exploring the relation between sulcular fluid volume and peri-implant condition.Methods Forty implants and matched healthy teeth in 29 patients were included and divided into implant group and normal group.The implant group was again divided into healthy group and inflammatory group according to well-defined clinical and radiographic criteria,fifteen implants in healthy group,twenty-five implants in inflammatory group.PISF was collected by paper strip and then the volume of PISF and gingival crevicular fluid(GCF) were estimated.Results The difference of PISF volume,plaque index(PLI),modified sulcus bleeding index(MBI)and probing depth(PD) among normal group,healthy implant group and inflammatory implant group were statistically significant(P<0.001).The volume of PISF and clinical parameters were higher(P<0.05) in inflammatory implant group than in normal group and healthy implant group,and no significant difference was found between normal group and healthy implant group(P<0.05).The PISF volume was associated with plaque index(r=0.631,P=0.001),modified sulcus bleeding index(r=0.502,P=(0.012)) and probing depth(r=0.546,P=0.006).Conclusion The PISF volume can reflect the degree of inflammatory peri-implant disease,could be helpful in early diagnosis for peri-implantitis.
