ERCP在治疗胆总管结石中的应用

目的:评价ERCP在治疗与诊断胆总管结石中的应用效果。方法:选择因上腹疼痛或腹胀的病例84例,术前对可疑胆总管结石患者均行MRCP和ERCP检查,术后随访6～12个月。结果:84例患者中,其中非胆总管结石可疑患者44例,胆总管结石可疑患者40例,MRCP共检查出结石25例,ERCP检查出结石23例,同时行术治疗证实这23例存在胆总管结石;两种方法检查特异性与敏感性都比较高,两组间比较,差异无统计学意义(P>0.05)。结论:胆总管结石可疑患者首先行MRCP筛选,阳性者进一步ERCP检查,能确保治疗的有效率,值得临床推广。

The applying of ERCP in the treatment of common bile duct stones

Objective To evaluate the applying effects of ERCP in the treatment of common bile duct stones.Methods Seleceted 84 cases of patients with abdominal pain or bloating,preoperative suspected choledocholithiasis patients were underwent MRCP and ERCP examination,and then were followed up for 6～12 months.Results There were 44 cases with non-suspected common bile duct stones and 40 cases with suspected common bile duct stones,MRCP detect out the stones were 25 cases,ERCP detect out the stones were 23 cases,while the line cut stone duodenal papilla confirm the existence of common bile duct stones in 23 cases;two ways to detect the specificity and sensitivity were high,the two groups showed no significant difference（P0.05）.Conclusion Patients with suspected choledocholithiasis should directly undergolaparoscopic cholecystectomy,The patients with suspected common bile duct stones MRCP screened positive were further ERCP examination for treating effectiveness.
