| 摘 要： | 本文总结近几年血管内球囊栓塞治疗24例外伤性颈内动脉-海绵窦瘘的体会,其中21例外伤性颅底骨折,3例为火器伤引起.均采用经股动脉血管内球囊栓塞治愈,单纯瘘口闭塞14例,颈内动脉通畅率为58%,无1例永久性并发症.作者认为栓塞时用等渗造影剂充盈球囊,力争用少数球囊单纯闭塞瘘口,如果颈内动脉侧支循环代偿好,也可直接使用两枚球囊闭塞颈内动脉治愈海绵窦瘘,同样可获满意的效果.

| Abstract: | The experience of endovascular em-bolization with detachable balloon in 24 patients of traumatic carotid-cavernous fistulas (TCCF) in the period of 1991-1995 was reported. Of the 24 patients, 21 were traumatic craniobasial fracture, 3 penetrative gun shot wound. All these patients of TCCF were cured by balloon embolization through the femoral artery approach. 14 cases with simple fistulas were successfully occluded. The patency rate of carotid artery was 58%, there was no permanent com-plication,The authors prefer to use the contrast medium for balloon inflation in order to employ a fewer balloons to occlude the fistulas. If the collateral circulation of carotid artery is adequate, then two balloons were placed in the carotid artery at the proximal and distal to the opening of the carotid-cavernous fistulas, the satisfactory result also can be obtained. |
