川芎茶调散合身痛逐瘀汤加减配合针灸治疗坐骨神经痛疗效研究

目的:探讨川芎茶调散合身痛逐瘀汤加减配合针灸治疗坐骨神经痛的疗效。方法:选取坐骨神经痛患者共153例,对照组75例接受常规西药及针灸治疗,观察组78例在其基础上加用川芎茶调散合身痛逐瘀汤加减治疗,比较两组临床总有效率、神经传导速度、疼痛程度评分及致痛物质水平。结果:观察组总有效率89.74%(70/78)显著高于对照组73.33%(55/75,P<0.05);观察组腓肠神经感觉神经传导速度SCV、腓肠神经运动神经传导速度MCV及病变侧胫神经运动神经传导速度MCV显著高于对照组(P<0.05);观察组腰骶部、臀部、大腿外侧、腘窝、小腿外侧VAS评分显著低于对照组(P<0.05);观察组血浆5-羟色胺(5-HT)和P物质(SP)水平显著低于对照组(P<0.05)。结论:川芎茶调散合身痛逐瘀汤加减配合针灸治疗坐骨神经痛具有良好的临床疗效,能够对有效提高患者神经传导速度,减少致痛物质的合成及分泌,缓解疼痛。

Curative effect of Chuanxiong Chatiaosan and Modified Shengtong Zhuyu decoction combined with acupuncture on sciatica

Objective:To explore curative effect of Chuanxiong Chatiaosan and modified Shengtong Zhuyu decoction combined with acupuncture on sciatica.Methods:78 sciatica patients who were admitted to rehabilitation department were enrolled as observation group.75 sciatica patients who were admitted to rehabilitation department were enrolled as control group.The control group was given routine western medicine and acupuncture treatment.On this basis,observation group was additionally given Chuanxiong Chatiao san and modified Shengtong Zhuyu decoction.The total clinical response rate,nerve conduction velocity,pain degree score and levels of pain-causing substances were compared between the two groups.Results:The total response rate of observation group was significantly higher than that of control group89.74%(70/78)vs 73.33%(55/75)](P<0.05).The sensory conduction velocity(SCV)of sural nerve,motor conduction velocity(MCV)of sural nerve,and MCV of tibial nerve at lesion side in observation group were significantly higher than those in control group(P<0.05).VAS scores at lumbosacral region,buttock,lateral thigh,popliteal fossa,and lateral calf in observation group were significantly lower than those in control group(P<0.05).The levels of plasma serotonin(5-HT)and substance P(SP)in observation group were significantly lower than those in control group(P<0.05).Conclusion:The clinical curative effect of Chuanxiong Chatiao san and modified Shengtong Zhuyu decoction combined with acupuncture is good on sciatica,which can effectively improve nerve conduction velocity,reduce synthesis and secretion of pain-causing substances,and relieve pain.
