自拟胃复汤对不同证型消化性溃疡胃泌素和血液流变学的影响

目的探讨自拟胃复汤对不同证型消化性溃疡胃泌素和血液流变学的影响。方法选取我院2017年7月-2018年9月收治的78例消化性溃疡患者作为研究对象,其中胃络瘀阻证22例,脾胃湿热证28例和脾胃虚寒证28例。对患者治疗前后的胃泌素水平及血沉、血细胞比容等血液流变学指标进行检测和比较分析。结果胃泌素组间比较,治疗前3个证型组有一定差别,其中胃络瘀阻证组明显高于脾胃虚寒证组(p<0.05),3组治疗后趋于一致。组内比较:治疗6周后3个证型组均较治疗前明显下降(p<0.05),以胃络瘀阻证患者下降幅度最大;血细胞比容组间比较,治疗前3组大致相近,仅胃络瘀阻证较高于脾胃虚寒证组(p<0.05)。治疗后3组差别较大,其中胃络瘀阻证组均明显低于另外2组(p<0.05)。组内比较,胃络瘀阻证和脾胃湿热证组下降幅度较大,差异有统计学意义(p<0.05);血沉水平组间比较,治疗前3证型组相近(p>0.05),治疗后(6周)胃络瘀阻证组均明显低于另外2组(p<0.05)。组内比较,仅胃络瘀阻证组下降幅度较大,治疗前后比较差异有统计学意义。结论自拟胃复汤联合奥美拉唑可有效减少胃泌素的生成,改善血液流变学指标,对于胃络瘀阻证、胃湿热证以及脾胃虚寒证均具有显著的疗效,其中对于胃络瘀阻证胃泌素和血液流变学指标的影响最为突出。

Effect of Self-made Weifu Decoction on Gastrin and Hemorheology of Different Types of Peptic Ulcer

Objective To investigate the effects of self-made Weifu decoction combined with omeprazole on gastrin and hemorheology of peptic ulcer with different syndromes.Methods To select 78 cases of peptic ulcer patients admitted to our hospital from July 2017 to September 2018 as research objects,including 22 cases of stomach collature-stasis syndrome,28 cases of spleen and stomach damp-heat syndrome,and 28 cases of spleen and stomach deficiency and cold syndrome.Then compared and analyzed the levels of gastrin and hemorheological indexes such as erythrocyte sedimentation and hematocrit.Results Gastrin:The comparison among groups showed there were some differences among the three groups before treatment,among which the group of stomach collature-stasis syndrome was significantly higher than the group of spleen and stomach deficiency and cold syndrome(P<0.05),and the three groups tended to be consistent after treatment.Intra-group comparison showed that after 6 weeks of treatment,all the three syndrome groups showed a significant decrease compared with that before treatment(P<0.05),with the largest decrease in patients ofstomach collature-stasis syndrome.Hematocrit:The comparison between groups showed that the three groups before treatment were roughly similar,with only the stomach collature-stasis syndrome was higher than that of the group of spleen and stomach deficiency and cold syndrome(P<0.05).After treatment,there was a significant difference among the three groups,among which the stomach collature-stasis syndrome group was significantly lower than the other two groups(P<0.05).Intra-group comparison showed the decrease of stomach collature-stasis syndrome and spleen and stomach deficiency and cold syndrome were large,and the difference in treatment was significant(P<0.05).Blood sedimentation level:The comparison between groups showed the three syndromes were similar before treatment(P>0.05),and the stomach collature-stasis syndrome group after treatment(6 weeks)was significantly lower than the other two groups(P<0.05).Intra-group comparison showed only the stomach collature-stasis syndrome group showed a significant decrease,and the difference before and after treatment was statistically significant.Conclusion The self-designed Weifu decoction combined with omeprazole can effectively reduce the generation of gastrin and improve hemorheology indexes,which had a significant effect on the stomach collature-stasis syndrome,spleen and stomach damp-heat syndrome and spleen and stomach deficiency and cold syndrome,among which gastrin and hemorheology indexes had the most prominent effect on the syndrome of stomach collature-stasis syndrome.
