麻醉的中医作用理论及瑞芬太尼与芬太尼在高龄患者静脉麻醉中的疗效

目的:探讨麻醉的中医作用理论及瑞芬太尼与芬太尼在高龄患者静脉麻醉中的疗效。方法:将2017年11月-2018年11月在单县中医医院普外科手术治疗的78例高龄患者随机分为两组,均采用全身静脉麻醉,对照组使用芬太尼静脉麻醉,观察组使用瑞芬太尼静脉麻醉,比较两组患者的不同时间点血流动力学指标变化、术后苏醒质量。结果:观察组麻醉诱导前、诱导后、气管插管时、气管拔管时收缩压(SBP)、舒张压(DBP)、平均压(HR)均较对照组控制平稳(P<0.05);观察组术后苏醒时间、拔管时间、认知功能恢复时间均明显短于对照组(P<0.05)。结论:在中医理论指导下,瑞芬太尼在高龄患者静脉麻醉中的疗效更佳,术中血流动力学更为平稳,术后苏醒速度更快,苏醒质量更高,具有积极的临床意义。

Theory of traditional Chinese medicine on anesthesia and the efficacy of remifentanil and fentanyl in intravenous anesthesia for elderly patients

Objective:To explore the theory of traditional Chinese medicine on anesthesia and the efficacy of remifentanil and fentanyl in intravenous anesthesia for elderly patients.Methods:78 elderly patients who were treated with general surgery in our hospital from November 2017 to November 2018 were randomly divided into two groups,all of which received general intravenous anesthesia,the control group received intravenous fentanyl anesthesia,and the observation group received intravenous remifentanil anesthesia.Hemodynamic index changes and postoperative recovery quality of patients in the two groups were compared at different time points.Results:SBP,DBP and HR in the observation group were more stable before and after anesthesia induction,during tracheal intubation,and during tracheal extubation than in the control group(P<0.05).The postoperative recovery time,extubation time and cognitive function recovery time of the observation group were significantly shorter than those of the control group(P<0.05).Conclusion:Under the guidance of the theory of traditional Chinese medicine,remifentanil has better efficacy in intravenous anesthesia of elderly patients,it has more stable intraoperative hemodynamics,faster postoperative recovery rate and higher quality of recovery,which has positive clinical significance.
