天麻素联合异丙嗪与单用天麻素比较治疗眩晕症Meta分析

目的探讨天麻素联合异丙嗪治疗眩晕症的有效性和安全性,为其临床推广提供循证医学依据。方法计算机检索中国生物医学文献数据库(CBM),万方数据库,中国科技期刊全文数据库(VIP),中国期刊全文数据库(CNKI),PubMed,The Cochrane Library和Embase数据库,搜集天麻素联合异丙嗪治疗眩晕症的随机对照试验,检索时间均为建库至2019年6月12日。由两名研究者按照纳入与排除标准独立筛选文献、提取资料并评价质量后,采用RevMan5.3软件进行Meta分析。结果共纳入13篇随机对照研究,共884例患者。Meta分析结果显示,与单纯应用天麻素相比,应用天麻素联合异丙嗪可以显著提高眩晕症治疗的临床有效率[OR=6.97,95%CI(4.18,11.61),P<0.00001],缩短起效时间[SMD=-2.72,95%CI(-3.86,-1.58),P<0.00001],并且减少不良反应的发生[OR=0.21,95%CI(0.04,0.98),P=0.05]。结论天麻素联合异丙嗪比单独应用天麻素治疗眩晕症有更好的疗效和安全性,但由于纳入文献质量较低,需要高质量的大样本双盲随机对照试验的支持。

Meta-analysis of Gastrodin Combined with Promethazine on Vertigo

Objective:To investigate the efficacy and safety of gastrodin combined with Promethazine on vertigo,and to provide evidences of evidence-based medicine for its clinical promotion.Methods:CBM,Wanfang Database,VIP,CNKI,PubMed,The Cochrane Library and Embase Database were searched with computer to collect RCTs of gastrodin combined with Promethazine on vertigo.The retrieval time was from the establishment of the database to June 12,2019.Two researchers independently selected literature according to the inclusion and exclusion criteria,extracted data and evaluated the quality,and then used RevMan 5.3 software for meta-analysis.Results:13 RCTs and 884 patients were included.Meta-analysis showed that the application of gastrodin combined with Promethazine significantly improved the clinical efficacy compared with gastrodin alone[OR=6.97,95%CI(4.18,11.61),P<0.00001],shortened the onset time[SMD=-2.72,95%CI(-3.86,-1.58),P<00001],and reduced the occurrence of adverse reactions[OR=0.21,95%CI(0.04,0.98),P=0.05].Conclusion:Gastrodin combined with Promethazine has better efficacy and safety than gastrodin alone on vertigo.However,due to the low quality of the included literature,high quality large-sample double-blind randomized controlled trials are needed.
