复方丹参滴丸对急性心肌梗死后心力衰竭模型家兔心功能的影响

摘    要:目的观察复方丹参滴丸对家兔急性心肌梗死后心力衰竭模型离体心脏离子通道和心功能的影响。方法采用随机、对照、盲法原则将40只家兔均分为模型组、硝苯地平组和复方丹参滴丸组。采用垂体后叶素(2 U/kg)耳缘静脉注射诱发急性心肌缺血,并叠加冠状动脉左前降支结扎术建立家兔急性心肌梗死后心力衰竭模型。用Langendorff灌流系统观察家兔离体心脏心功能指标中左室内压力峰值(LVSP)、左室射血分数(LVEF)、等容收缩期左心室内压力上升/下降最大速率(+dp/dtmax/-dp/dtmax)。原子吸收光谱法检测冠状动脉左前降支缺血心室肌组织间液钠(Na^+)、钾(K^+)和钙离子(Ca^2+)的浓度。采用酶联免疫吸附测定法(ELISA)检测血清心肌肌钙蛋白T(cTnT)、心肌肌钙蛋白I(cTnI)和肌钙蛋白C(TnC)和心肌钙调蛋白依赖性蛋白激酶Ⅱ(CaMKⅡ)表达。结果与模型组和硝苯地平组比较,复方丹参滴丸组心肌组织间液中Na+浓度降低,K^+和Ca^2+浓度升高(P<0.05),血清cTnT、cTnI和TnC蛋白及心肌CaMKⅡ均明显低表达(P<0.05)。其心功能与模型组比较,-dp/dtmax降低,+dp/dtmax、LVSP和LVEF均升高(P<0.05),但与硝苯地平组比较无显著差异(P>0.05)。结论复方丹参滴丸能在一定程度上改善急性心肌梗死后心力衰竭模型家兔心功能。

Abstract:Objective:To observe the effect of Compound Danshen Dropping Pills(CDDP)on the ion channel and cardiac function of isolated heart after acute myocardial infarction in rabbits.Methods:40 rabbits were randomly divided into the model group,nifedipine group and CDDP group(n=10).Acute myocardial ischemia in rabbits was induced by injection of 2 U/kg(pituitrin)and the left anterior descending branch of coronary artery was ligated to establish the model of heart failure after acute myocardial infarction in rabbits.LVSP,LVEF and the maximum rate of left ventricular pressure rising/descending at the maximum rate of(+dp/dtmax/-dp/dtmax)were observed by Langendorff perfusion system in the isolated cardiac cardiac function index of rabbits.The concentrations of Na^+,K^+ and Ca^2+ in the interstitial fluid of the ischemic left anterior descending coronary artery were measured by atomic absorption spectrometry.Serum cTnT,cTnI,TNC and CaMKⅡwere detected by ELISA.Results:Compared with the model group and nifedipine group,the concentration of Na^+ in the myocardial interstitial fluid of CDDP group decreased,and that of K+and Ca^2+ increased(P<0.05).The serum cTnT,cTnI,TnC protein and myocardial CaMKⅡobviously presented low expression(P<0.05).Compared with the model group,-dp/dtmax decreased,and+dp/dtmax,LVSP and LVEF increased(P<0.05),but compared with nifedipine group,there was no significant difference(P>0.05).Conclusion:Compound Danshen Dropping Pills can obviously improve the heart function of rabbits with heart failure after acute myocardial infarction.
