中医方药治疗心肌缺血再灌注损伤的研究进展

再灌注是治疗急性心肌梗死最有效的手段,通过早期药物溶栓、介入治疗等方式使缺血区心肌进行血运重建。然而恢复灌注的血流供应却使得心肌组织二次受损,即心肌缺血再灌注损伤。近年来的实验研究表明,中医方药可以通过药物预处理的方式,减缓因再灌注导致的心肌损伤;通过清除多余的氧自由基,调节ATP的细胞膜泵活性,改善再灌注后心肌的能量代谢障碍;通过调控相关心肌细胞凋亡蛋白或炎性蛋白的表达,抑制细胞凋亡或炎症反应,挽救濒死的心肌细胞;通过抑制心肌自噬的表达以及调控相关缝隙连接蛋白的表达,从而减轻心室重构,达到保护受损心肌的目的。

Research Progress of Traditional Chinese Medicine on Myocardial Ischemia-Reperfusion Injury

Reperfusion is the most effective means for the treatment of acute myocardial infarction.The ischemic myocardium is revascularized by early drug thrombolysis and interventional therapy.However,the replenishment of the perfused blood flow supply causes secondary damage to the myocardial tissue,namely myocardial ischemiareperfusion injury.In recent years,experimental studies have shown that traditional Chinese medicine can slow the myocardial damage caused by reperfusion by means of drug pretreatment;regulate the cell membrane pump activity of ATP by removing excess oxygen free radicals,and improve the energy metabolism disorder of myocardial after reperfusion.By inhibiting calcium influx,preventing sodium-calcium ion exchange and hydrogen ion efflux,the uncontrolled mechanism of reperfusion arrhythmia can be regulated;by regulating the expression of related cardiomyocyte apoptosis protein or inflammatory protein,inhibition of apoptosis or inflammation The reaction saves the dead cardiomyocytes;inhibits the expression of autophagy and regulates the expression of related connexins,thereby reducing ventricular remodeling and achieving the purpose of protecting the damaged myocardium.
