中医针灸联合推拿手法治疗椎动脉型颈椎病的效果及对血流动力学的影响

目的:探讨中医针灸联合推拿手法治疗椎动脉型颈椎病(Cervical Spondylosis of Vertebral Artery Type,CSA)的效果及对血流动力学的影响。方法:选取符合入组条件病例数84例,来源于我院2017年1月-2019年1月收治的CSA患者,按随机平均对照原则分为对照组(42例,中医针灸)及观察组(42例,中医针灸联合推拿手法),对比两组患者血流动力学改变。结果:观察组治疗后时间平均流速(TMFV)、收缩期峰值流速(Peak Systdic Velocity,PSV)高于对照组,检测血管搏动指数(PI)、阻力指数(RI)低于对照组,差异有统计学意义(P<0.05)。观察组治疗后中医临床症状评分(3.08±0.43)分低于对照组(5.16±0.71)分,差异有统计学意义(P<0.05)。两组患者治疗期间未出现脉搏、意识、心率、感觉等不良反应,无脊髓或椎动脉损伤,观察组未出现晕针、断针等不良影响。结论:中医针灸联合推拿手法治疗椎动脉型颈椎病获得显著效果,利于血流动力学改善,临床价值高,值得推广。

Efficacy of acupuncture plus massage on CSA and hemodynamics

Objective:To investigate clinical efficacy of acupuncture plus massage on CSA and hemodynamics.Methods:84 patients were randomly divided into two groups,42 patients in each.The control group was given acupuncture,and the observation group was given acupuncture plus massage.Results:After treatment,in the observation group,TMFV and PSV were higher,and PI and RI were lower than the control group,the differences were statistically significant(P<0.05).The score of TCM clinical symptoms was lower than the control group(P<0.05).During the treatment,there was no adverse reactions in pulse,consciousness,heart rate,and sensation in both groups,no spinal or vertebral artery damage.There was no adverse reactions such as halo and broken needle in the observation group.Conclusion:Acupuncture plus massage on CSA show significant effect,can improve hemodynamics,and is worthy of promotion.
