针刺配合补阳还五汤治疗脑梗死的临床疗效分析

目的:分析针刺配合补阳还五汤治疗脑梗死的临床疗效。方法:选择我院在2018年1-12月收治的120例脑梗死患者,依据治疗方法将其分为对照组(60例)与观察组(60例),对照组患者应用补阳还五汤治疗,观察组患者应用针刺配合补阳还五汤治疗,对比分析两组患者的治疗效果、治疗前后的神经功能损伤评分、日常生活能力、肢体运动功能评分,血液流变学各项指标变化情况、血浆血管内皮生长因子(Vascular Endothelial Growth Factor,VEGF)、神经肽Y(Neuropeptide Y,NPY)含量。结果:观察组治疗总有效率高于对照组;治疗前的神经缺损评分量表(National Institute of Health Stroke Scale,NIHSS)评分、日常生活活动评分(采用Barthel指数)及Fugl-Meyer运动功能两组对比差异不明显(P>0.05),但治疗后观察组NIHSS评分低于对照组,Barthel指数与Fugl-Meyer运动功能均高于对照组(P<0.05);观察组患者治疗后的血浆粘度、全血粘度与纤维蛋白原均低于对照组(P<0.05);两组患者治疗前血浆VEGF与NPY含量对比差异不明显,治疗后观察组血浆VEGF含量高于对照组,且血浆NPY含量低于对照组(P<0.05)。结论:针对脑梗死采用针刺配合补阳还五汤治疗效果显著,能够缓解临床症状,改善神经功能损伤情况,有利于提高患者的生活质量,具有临床应用价值。

Clinical effects of acupuncture plus the Buyang Huanwu decoctionon cerebral infarction

Objective:To analyze the clinical efficacy of acupuncture plus the Buyang Huanwu decoction(补阳还五汤)on cerebral infarction.Methods:According to the treatment,120 patients were divided into the control group(60 cases)and observation group(60 cases).The control group was treated with the Buyang Huanwu decoction.The observation group was treated by acupuncture more.The effects,neurological impairment scores,daily living ability,limb motor function scores,and blood rheology plasma VEGF,NPY content before and after treatment were compared and analyzed.Results:The total efficiency in the observation group was higher than the control group.There was no significant difference in NIHSS score,Barthel index and Fugl-Meyer motor function before treatment between groups(P﹥0.05).After treatment,the NIHSS score in the observation group was lower,and the Barthel index and Fugl-Meyer motor function were higher than the control group(P﹤0.05).The plasma viscosity,whole blood viscosity and fibrinogen in the observation group were lower than the control group(P﹤0.05).There was no significant difference in plasma VEGF and NPY levels before treatment between the two groups.After treatment,the plasma VEGF level in the observation group was higher than the control group,and the plasma NPY content was lower than the control group(P﹤0.05).Conclusion:Acupuncture plus the Buyang Huanwu decoction on cerebral infarction shows significant effects,can alleviate clinical symptoms,improve neurological impairment and the quality of life,and have clinical application value.
