基于数据挖掘技术探讨针刺治疗缺血性视神经病变的选穴规律

摘    要:目的分析并总结针刺治疗缺血性视神经病变的选穴规律。方法采用以"针刺"或"针灸"或"针"和"缺血性视神经病变"为主题词的检索方式,收集中国期刊全文数据库(CNKI)、万方数据库(WF)、维普数据库(VIP)、中国生物医学文献数据库(CBM)、Pubmed、Web of Science和Embase中针刺治疗缺血性视神经病变的临床文献,用Excel 2010软件建立腧穴数据库,运用数据挖掘技术中的聚类分析和关联规则分析方法,分析针灸处方用穴的特点和规律。结果针刺治疗缺血性视神经病变以足太阳膀胱经、足少阳胆经和足阳明胃经的腧穴及经外奇穴为主,常用腧穴主要分布于眼周,交会穴、原穴和五腧穴等特定穴应用广泛,关联规则分析显示腧穴间相关性最高的是合谷-太阳、合谷-风池,通过聚类分析得出4个有效聚类群:视区-三阴交、瞳子髎-丝竹空、球后-睛明-风池、太阳-合谷。结论现代针刺治疗缺血性视神经病变尤其重视合谷-太阳、合谷-风池与其他腧穴配伍,体现"经脉所过,主治所及""病位所在,主治所在"的选穴思想,采用远近配穴、上下配穴的取穴原则,重用特定穴。

Abstract:Objective:To determine acupoint selection rules on ischemic optic neuropathy based on data mining. Methods:Papers were retrieved from databases CNKI,WF,VIP,CBM,Pubmed,Web of Science and Embase by using the keywords acupuncture and ischemic optic neuropathy. The main points of acupuncture were established with Excel 2003 software;the characteristics and laws in acupuncture were analyzed with the relevance rule and cluster analysis Methods in data mining technology. Results:Acupuncture treatment of ischaemic optic neuropathy mainly included the acupoints and extra meridian acupoints of Bladder Meridian of Foot-Taiyang,Gallbladder Meridian of Foot-Shaoyang and Stomach Meridian of Foot-Yangming. Common acupoints were mainly distributed around the eyes. Specific points like convergont points,Yuan-source acupoints and Wu-shu acupoints were widely applied. The analysis of association rules showed that Hegu and Taiyang,Hegu and Fengchi had the highest correlation among acupoints. Four effective clustering groups are obtained by cluster analysis:visual areaSanyinjiao,Tongziliao-Sanyinjiao,Tongziliao-Sizhukong,Qiuhou-Jingming-Fengchi,Taiyang-Hegu. Conclusion:Modern acupuncture treatment of ischemic optic neuropathy pays special attention to the compatibility of HeguTaiyang and Hegu-Fengchi with other acupoints,embodying the idea that"where meridians pass,where diseases are located,where they are treated",and adopting the principles of distant and near acupoints matching,upper and lower acupoints matching,and reusing specific acupoints.
