健脾益肾防滑汤治疗脾肾两虚型复发性流产的临床研究

目的:观察健脾益肾防滑汤对脾肾两虚型复发性流产再次妊娠率及妊娠结局的影响情况;通过中医证候对比观察防滑汤的疗效情况;观察健脾益肾防滑汤对孕后雌二醇(E2)、孕酮(Progesterone,P)、血人绒毛膜促性腺激素(Human Chorionic Gonadotropin,HCG)数值的影响。方法:选取在我院门诊或住院经确诊为脾肾两虚型复发性流产患者60例,随机分为对照组30例,治疗组30例。治疗组:准备妊娠前3个月口服健脾益肾防滑汤,服药时间:月经净后服用,2周为一个疗程,连服3个疗程。孕后加服地屈孕酮10 mg,每日3次,口服,至孕12周停服。对照组:孕前不服药,孕后服用地屈孕酮10 mg,每日3次,口服,至孕12周停服。结果:两组总有效率比较,P<0.05,差异有统计学意义。对照组正常妊娠率低于治疗组,P<0.05,差异有统计学意义。两组孕40 d及孕60 d,两组自身血清P、HCG、E2比较,P<0.01,差异有统计学意义;对照组与治疗组孕60 d比较,P<0.01,差异有统计学意义。结论:健脾益肾防滑汤可提高脾肾两虚型复发性流产再次妊娠率,可改善脾肾两虚型复发性流产患者再次妊娠结局,可提高脾肾两虚型复发性流产孕后E2、P、血HCG水平。

Observation on treating recurrent spontaneous abortion of the Pishen Liangxu type with the Jianpi Yishen Fanghua decoction

Objective:To observe the effect of the Jianpi Yishen Fanghua decoction(健脾益肾防滑汤)on the re-pregnancy rate,pregnancy outcome,E2,P and HCG in patients with recurrent spontaneous abortion of the Pishen Liangxu type(脾肾两虚型),to compare TCM syndromes and the effects.Methods:60 patients were randomly divided into the control group and treatment group,30 patients in each group.The treatment group was treated with the Jianpi Yishen Fanghua decoction before preparing 3 months and dydrogesterone after pregnancy to 12 weeks.The control group was not given medicine before pregnancy,treated with dydrogesterone after pregnancy to 12 weeks.Results:The total effective rate was lorrer than that in the treatment group,P<0.05,with significant differences.The normal pregnancy rate was lorrer than that in the treatment group,P<0.05.There were significant differences of serum P,HCG and E2 in groups compared with 40 days and 60 days of gestation.There were significant differences of serum P,HCG and E2 in groups compare with 60 days of gestation,P<0.01.Conclusion:The Jianpi Yishen Fanghua decoction can improve the rate of re-pregnancy on recurrent spontaneous abortion of the Pishen liangxu type,and can improve pregnancy outcome,E2,P,HCG levels.
