手法闭合复位结合交锁髓内钉治疗股骨干骨折的临床疗效研究

目的:观察手法闭合复位结合交锁髓内钉治疗股骨干骨折的临床疗效,总结临床应用价值。方法:选择自2016年1月-2019年1月来我院骨科接受治疗的股骨干骨折患者68例,根据随机数字法分为对照组和观察组,各34例,对照组采用切开复位加锁髓内钉治疗,观察组采用手法闭合复位结合交锁髓内钉治疗,观察对比患者的术中出血量和骨折愈合时间,手术前后的视觉模拟评分法(Visual Analogue Scale,VAS)评分,术后第6个月的髋关节优良率、临床总有效率和术后6个月内的并发症发生率。结果:与对照组比较,观察组术中出血量和骨折愈合时间、出院当天的VAS评分、术后6个月内并发症发生率均明显更低,术后第6个月观察组的髋关节优良率和临床总有效率均明显更高,(P<0.05),具有统计学意义。结论:手法闭合复位结合交锁髓内钉治疗股骨干骨折,能够降低患者术中出血量,促进骨折愈合,减少术后疼痛和并发症发生率,治疗效果确切,值得临床推广应用。

Clinical efficacy of manipulation closed reduction plus interlocking intramedullary nail on femoral shaft fracture

Objective:To observe clinical efficacy of manipulation closed reduction plus interlocking intramedullary nail on femoral shaft fracture.Methods:68 cases were divided into two groups,34 cases in each.The control group was given open reduction and interlocking intramedullary nail,and the observation group was given manipulation closed reduction plus interlocking intramedullary nail.Results:Compared with the control group,the amount of bleeding and fracture healing time,VAS score on the day of discharge and the incidence of complications within 6 months after operation were significantly lower in the observation group.At the 6 th month after operation,the hip joint excellent rate and clinical efficiency were significantly higher in the observation group(P﹤0.05).Conclusion:Manipulation closed reduction plus interlocking intramedullary nail on femoral shaft fracture shows good effects,can reduce the amount of bleeding during the operation,promote fracture healing,reduce the incidence of postoperative pain and complications,and is worthy of promotion.
