面部电针联合双下肢阴、阳经温针灸治疗周围性面瘫临床研究

目的基于《灵枢经·终始第九》理论,观察电针结合双下肢阴、阳经温针灸治疗周围性面瘫疗法的临床疗效,为疾病治疗提供新的思路和方法。方法将80例周围性面瘫患者随机分为治疗组和对照组,每组各40例。对照组采用常规单纯面部电针疗法,治疗组在对照组电针治疗基础上在双下肢根据患者的人迎、寸口脉大小选择小腿内侧或外侧的穴位进行温针灸,同时浅刺双足的足通谷、京骨、束骨等穴,每日1次,5 d为1个疗程,共4个疗程,疗程间隔2 d。在治疗前后用House Brackmann(H-B)面神经功能评价量表对两组患者进行评分,并评价临床疗效,并通过比较治疗后两组患者的满意度及生活质量来评估两种疗法的疗效及差异是否显著。结果治疗后两组H-B分级及生活质量SF-36量表评分均较本组治疗前改善,治疗组改善程度优于对照组(P<0.05)。经治疗后,治疗组总有效率为92.50%,高于对照组总有效率的80.00%,差异有统计学意义(P<0.05),治疗组疗效明显优于对照组。同时治疗组患者满意度为85.00%,明显高于对照组的72.50%(P<0.05)。结论电针结合双下肢阴、阳经温针灸治疗周围性面瘫疗法疗效显著,优于常规单纯面部电针疗法,可以为临床上针灸治疗周围性面瘫提供一种新方法、新思路。

Clinical Observation on Electric Acupuncture Combined with Needle Warming Moxibustion on Peripheral Facial Paralysis by Yin and Yang Meridians of Both Lower Limbs

Objective:To observe the clinical effect of electro-acupuncture combined with needle warming moxibustion of Yin and Yang meridians of both lower limbs on peripheral facial paralysis.Methods:80 patients were randomly divided into the treatment group and the control group,40 cases in each.The control group was treated with routine facial electro-acupuncture alone.The treatment group was treated with electro-acupuncture on the basis of the control group.On the basis of electro-acupuncture,the lower extremities were treated with needle warming moxibustion at the inner or outer points of the lower leg according to the power of the patient′s Renying and Cunkou pulse.At the same time,the feet were shallowly pricked at Zutonggu,Jinggu and Shugu points once a day for 5 days as a course,with 4 treatment courses,with an interval of 2 days.House Brackmann(H-B)Facial Nerve Function Assessment Scale was used to evaluate the clinical efficacy before and after treatment;by comparing the satisfaction and quality of life of two groups of patients after treatment,the efficacy and difference of the two treatments were evaluated.Results:After treatment,H-B grading of both groups was better than that before treatment,and the treatment group was better than the control group(P<0.05).The total effective rate of the treatment group was 92.50%,and that of the control group was 80.00%.The difference was statistically significant(P<0.05).The curative effect of the treatment group was significantly better than that of the control group.The satisfaction of the patients in the treatment group was 85.00%,significantly higher than that in the control group(72.50%)(P<0.05).Conclusion:Electro-acupuncture combined with needle warming moxibustion of Yin and Yang meridians of both lower limbs is effective on peripheral facial paralysis,superior to simple conventional facial electro-acupuncture therapy.It can provide a new method and new idea for clinical treatment of peripheral facial paralysis.
