天麻钩藤饮加味合醒脑静注射液治疗高血压性脑出血术后患者疗效研究

目的观察天麻钩藤饮加味合醒脑静注射液治疗高血压性脑出血术后患者疗效及对实验室指标水平的影响。方法患者共108例以随机抽签法分为对照组及观察组各54例,分别给予单纯西医对症干预和在此基础上加用天麻钩藤饮加味合醒脑静注射液治疗,比较两组近期疗效、治疗前后主要证候评分、美国国立卫生院卒中量表(NIHSS)评分、格拉斯哥昏迷指数量表(GCS)评分、Barthel指数(BI)评分、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、基质金属蛋白酶-9(MMP-9)、超氧化物歧化酶(SOD)、丙二醛(MDA)、人巨噬细胞炎性蛋白-1α(MIP-1α)及胶原纤维酸性蛋白(GFAP)水平。结果观察组近期疗效显著优于对照组(P<0.05);两组治疗后主要证候评分、NIHSS评分、GCS评分及BI评分均较治疗前改善(P<0.05),观察组改善程度显著优于对照组(P<0.05);观察组治疗后IL-6、TNF-α、MMP-9、SOD、MDA、MIP-1α及GFAP水平均较治疗前显著改善(P<0.05),观察组改善程度更为显著(P<0.05)。结论天麻钩藤饮加味合醒脑静注射液治疗高血压性脑出血术后患者可有效改善临床症状,保护神经系统功能,促进生活质量提高,并有助于调节炎性细胞因子、氧化应激指标、MIP-1α及GFAP水平。

The Effects of Tianma Gouteng Decoction Combined with Xingnaojing Injection on Patients with Hypertensive Cerebral Hemorrhage after Operation and the Level of Laboratory Indices

Objective:To observe the therapeutic effects of Tianma Gouteng Decoction and Xingnaojing Injection on patients with hypertensive cerebral hemorrhage and the influence on the level of laboratory indices.Methods:108 patients were randomly divided into the control group and the observation group,54 cases in each.The control group received Western medicine intervention;on that basis,the observation group received Tianma Gouteng Decoction plus Xingnaojing Injection.The following indices were compared:short-term effect,the scores of main syndrome,NIHSS,GCS and BI and the levels including IL-6,TNF-α,MMP-9,SOD,MDA,MIP-1αand GFAP.Results:The short-term effect of the observation group was significantly better than that of the control group(P<0.05).After treatment,the main syndrome scores,NIHSS scores,GCS scores and BI scores of both groups were improved(P<0.05),and those of the observation group were significantly better than those of the control group(P<0.05).The levels of IL-6,TNF-α,MMP-9,SOD,MDA,MIP-1αand GFAP in the observation group were significantly improved compared with those before treatment(P<0.05).Conclusion:Tianma Gouteng Decoction combined with Xingnaojing Injection can effectively improve the clinical symptoms,protect the nervous system function,improve the quality of life,and help to regulate the levels of inflammatory cytokines,oxidative stress,MIP-1αand GFAP.
