兔眼巩膜内铜异物的病理学观察

摘    要:目的 了解巩膜内铜异物的病理改变及对眼组织的影响,并和铁作比较。方法 28只兔眼分为4组,分别于巩膜层问植入纯铜片、合金铜片或铁片。术后2个月和3个月处死动物,眼球做HE染色和组织化学染色,部分标本做电镜观察和视网膜铜元素分析。结果 巩膜层间的纯铜异物引起急性化脓性炎症反应;铜可以透过包囊和巩膜沉积于脉络膜和视网膜。合金铜异物引起有限的慢性炎症反应、包囊外组织无明显铜染色和炎症反应,怛视网膜内可探及铜元素峰。铁异物以吞噬反应为主,铁质沉积于球壁各层。结论 位于巩膜层间的铜或铁异物透过巩膜向视网膜逐渐渗透。持续的巩膜间纯铜异物存留可引起铜质沉着症和跟内炎症。合金铜异物危害性小于纯铜。

Abstract:Objecti ve To observe the pathological changes caused by intrascleral cooper foreign body in rabbit eyes.MethodsRed cooper,brass and iron foreign bodies were placed into scleral pocket of 28 rabbit eyes for 2 or 3 months.The specimens of eyeball s were used for HE and histochemical stain,microscopic analysis and energy dispe rsive analysis of x-ray (EDAX).ResultsAcute suppurative inflammation was se en in all eyeballs with red cooper foreign bodies,and cooper deposited in choro id and retina across cyst and sclera.Limited chronic inflammation was induced by brass foreign bodies,and no cooper staining substances outside the cyst were se en.Cu peak could be detected in the retina in these eyeballs by using EDAX.Obvio us swallowing reaction of phagocytes was observed when the foreign body was iron .ConclusionCooper and iron can go through the sclera and deposit into choroi d and retina.Because of the probability of extensive chalcosis,red cooper foreig n bodies should be removed as early as possible.However,intrascleral brass forei gn bodies can be followed-up closely.
