肾脏动静脉通过时间在肝移植术前肾功能评估中的应用

目的 探讨超声造影检测肾脏动静脉通过时间(RAVIT)对肝移植患者术前肾功能评价的应用价值.方法 对我院102例肝移植患者术前行肾脏超声造影检查,据临床症状分为肝硬化代偿期组、肝硬化失代偿期组和肝肾综合征组;选取健康人45例为对照组.造影后对肾门处肾动静脉主干内造影剂到达时间(AT)进行测定和差值分析.结果 肾动脉到达时间(BAAT)、肾静脉到达时间(RVAT)及RAVIT在肝硬化代偿期组和对照组间无明显差异(P＞0.05);肝硬化失代偿期组RATT、RVIT及RAVTT较肝硬化失代偿期组和对照组延长(P＜0.05);肝肾综合征组RATT、RVIT、RAVIT在4组中最长(P＜0.05).结论 RATT、RVTT及RAVTT可作为评价肝移植患者术前肾功能的参考指标.

The preliminary study of renal arteriovenous transit time in evaluating renal function in patients before liver transplantation

Objective To study the value of the renal arteriovenous transit time(RAVTT) in evaluating renal function with contrast-enhanced ultrasound(CEUS) in patients with hepatic cirrhosis before liver transplantation.Methods One hundred and two patients with hepatic cirrhosis before liver transplantation were examined by CEUS,including 36 compensated cirrhosis patients,43 decompensated cirrhosis and 23 patients with hepatorenal syndrome(HRS).Forty-five health adults were in control group.After injection of SonoVue,the arrival time(AT) of the contrast agents echoes in the renal portal artery and renal portal vein was analyzed.Results There was no significant difference of RAAT,RVAT and RAVTT between control group and compensated cirrhosis group(P>0.05).However,RAAT,RVAT and RAVTT of decompensated cirrhosis group were longer than that of the control group and compensated cirrhosis group(P<0.05),RAAT,RVAT and RAVTT of hepatorenal syndrome(HRS) group were obviously longer than that of other three groups(P<0.05).Conclusion Renal arteriovenous transit time of CEUS could be used as a reference index in evaluating renal function before liver transplantation.
