Nd∶YAG激光治疗草莓状血管瘤

目的 探讨Nd:YAG激光照射治疗草莓状血管瘤的方法和疗效.方法 草莓状血管瘤患儿630例703处病灶.除8处位于睑缘的病灶,为避免对眼睛造成损伤,采用静脉全麻外,其余均不需麻醉.以功率7w、距离0.1～0.2cm连续Nd:YAG激光对病灶进行间隔、点状照射凝固,照射区面积直径0.2 cm,间隔0.3 cm,术中及时应用75%乙醇作局部物理降温,术后对治疗部位进行精心护理及茶子油涂抹,并观察其局部热反应和疗效.结果 3个月至2年的随访观察,703处瘤体全部消失,无一例复发.除19个病灶由于术后护理不当,形成局部皮肤瘢痕愈合外,其余684处病灶均未出现局部瘢痕和萎缩.结论 Nd:YAG激光照射治疗草莓状血管瘤安全有效.随着治疗后时间的延长,治疗部位与周围皮肤颜色无明显差异.

Nd:YAG Laser Radiation on Strawberry Hemangioma

Objective To investigate the curative effect of Nd: YAG laser radiation on strawberry hemangioma.Methods Totally 630 cases of childem patients with strawberry hemangioma was studied.They was not used anesthesia except 8 cases were adopted general intravenous anesthesia with lesion in margines palpebrales to avoid eye injury.Continuing utilization Nd: YAG laser radiation adopted the same laser output power of 7 W and distance of 0.1 to 0.2 cm.All 703 lesions were used laser radiation with punctual and interrupted solidification.The diameter of radiation volume was 0.2 cm with 0.3 cm of distance between them.Then physical hypothermia was used with 75% alcohol during perioperation.The nursing care and applying teaseed oil were conducted, the local thermal reaction and curative effect were observed.Results All 630 cases of child patients with strawberry hemangioma(703 lesions) were followed up for 3 months to 2 years, the tumor was vanished, none of them was recurred.Among them 19 lesions healed with scar of skin partly because of improper postoperative care, and 684 lesions healed without scar and atrophy.Conclusions Continuing utilization of Nd: YAG laser radiation for strawberry hemangioma is safe and effective.There is no difference between the color of treating and surrounding skin with time extending.
