阵发性心房颤动发作前后心率变异性分析

摘    要:目的 :探讨植物神经系统异常在阵发性心房颤动中的作用。方法 :采用动态心电图记录 31例孤立性心房颤动患者 2 4h心电信息 ,分析其心率变异性变化。结果 :心房颤动发作前SDNN、HF明显升高 ,LF、LF/HF明显降低 ,SDNN与阵发性心房颤动发作频率呈正相关 (r =0 .4 35 ,P <0 .0 1) ,LF/HF与阵发性心房颤动发作频率及持续时间呈负相关 (r=- 0 .4 5 1,P <0 .0 1;r=- 0 .4 6 8,P <0 .0 1)。结论 :阵发性心房颤动发作前交感 -迷走神经平衡向着迷走神经张力增高 ,交感神经张力消退的方向偏移 ,使心房颤动易于发作和持续

Abstract:Objective:To elucidate the role of autonomic nervous dysfunction during onset and progress of paroxysmal atrial fibrillation. Methods:Time domain and power spectral components of heart rate variability were measured with 24 hour dynamic ECG recording in 31 patients with paroxysmal atrial fibrillation. Results:SDNN,HF before onset increased than that after onset of paroxysmal atrial fibrillation. LF,LF/HF decreased remarkably. There existed a positive relationship between frequency of paroxysmal atrial fibrillation and SDNN before onset of paroxysmal atrial fibrillation (r=0.435, P <0.01).LF/HF negatively correlated with the frequency and length of paroxysmal atrial fibrillation(r=-0.451, P <0.01;r=0.468, P <0.01). Conclusions:The results suggest a shift of sympatho vagal balance towards vagal activation and sympathetic withdrawal, favoring the occurrence and persistence of paroxysmal atrial fibrillation.
