下胸椎微创徒手椎弓根置钉技术的初步研究

摘    要:目的:探讨微创徒手椎弓根置钉技术在下胸椎中应用的准确性研究。方法:2007年3月~2010年4月选定A组17例不需椎管减压胸腰段椎体骨折患者,术前CT三维重建及定位钢丝网辅助拍片确定椎弓根体表投影,做2cm切口,单纯依靠手感行徒手椎弓根钉置入,常规行骨折复位固定术。术后CT评价40枚下胸椎螺钉置入位置,统计相关的螺钉置入情况及并发症发生率,并与B组同期18例切开置入的40枚下胸椎椎弓根钉相比较。结果:A、B两组神经血管及内脏并发症发生率均为0,螺钉置入情况比较差异无统计学意义(P>0.05)。随访中未发现断钉、断棒及拔钉情况。结论:下胸椎微创徒手椎弓根置钉技术安全可靠,有诸多优点适合临床推广。
