腰椎间盘突出并发神经根管狭窄症的诊断与手术治疗（附152例临床分析）

在ＣＴ广泛应用于脊柱疾患诊断以前，腰椎间盘突出并发神经根管狭窄症（ＬＤＨＬＲＳ）易被误诊为单纯腰椎间盘突出症，手术效果不令人满意。对１９８５～１９９１年收治的ＬＤＨＬＲＳ１５２例，充分利用ＣＴ检查，根据不同狭窄情况采用不同手术方法。术后随访１年３个月～６年８个月，优良率为９０．２％。作者提出：（１）应对ＬＤＨＬＲＳ做出定性和定量诊断；（２）根据引起神经管根管狭窄的不同因素，将狭窄分为硬性狭窄、软性狭窄及混合性狭窄，并依此决定手术方式。

Diagnosis and treatment of lumbar disc hernia with lateral recess stenosis-A report of 152 cases

Before the application of CT in the diagnosis of spinal disease,lumbar disc hernia with lateral recess stenosis(LDHLRS) was often mistaken for single lumbar disc hernia. The effect of operation for LDHLRS was unsatisfactory. 152 cases of LDHLRS has been operaed in the hospital from 1985 to 1991. Using CT examination,various kinds of stenosis were treated in different types of operation.The follow-up period varied from 1 year and 3 months to 6 years and 8 months.and excellent rate was as high as 90. 2%. The author raises the following questions : (1)The correct diagnosis of LDHLRS in nature and in location of the diseased area were made. (2)According to the causes of resulting in lateral recess stenosis, it is classified stiff stenosis,soft stenosis and mixed stenosis.
