喉罩在腹腔镜异位妊娠手术中的应用

目的：观察比较喉罩（laryngeal mask airway,LMA）通气和气管插管（tracheal intubation,TT）在腹腔镜异位妊娠手术围术期对血流动力学的影响。方法：选择60例因异位妊娠拟行腹腔镜手术治疗的患者,随机分为LMA组和TT组,每组30例,监测并记录麻醉诱导前,喉罩置入或气管插管即刻,喉罩置入或气管插管后5 min,喉罩或气管导管拔除时的收缩压（SBP）,舒张压（DBP）,平均动脉压（MAP）和心率的变化及术后患者咽部不适或咽痛的发生率。结果：LMA组置入喉罩即时,置入喉罩后5 min及拔除喉罩时的SBP,DBP,MAP,和HR均明显低于气管插管组,术后咽部不适或咽痛的发生率显著低于TT组。结论：LMA对腹腔镜异位妊娠手术围术期血流动力学的影响更轻微,患者感觉舒适,更适合于腹腔镜异位妊娠这种短小的手术。

Application of Laryngeal Mask Airway in Laparoscopic Ectopic Pregnancy Operation

Objective:To observe and compare the effect of the laryngeal mask airway(LMA)ventilation and otracheal intubation(TT)on the hemodynamic during the laparoscopic ectopic pregnancy operation perioperative.Methods:60 patients who will do laparoscopic ectopic pregnancy operation were choosed,randomly divide into LMA group and TT group,30 patients in each group.The following contents were monitored and recorded including contraction pressure,diastolic pressure,mean arterial pressure,heart rate change and pharyngeal discomfort or sore incidence in the moment including before induction of anesthesia,laryngeal mask placed or TT intubation instantly,LMA or TT intubation after 5 min,laryngeal mask or endotracheal tube was removed.Results:LMA placed instant,inserting the LMA after 5 min and the laryngeal mask removed,SBP,DBP,MAP,and HR were distinctly lower than TT group,postoperative pharyngeal discomfort or sore incidence was significantly lower than TT group.Conclusion:The LMA has slightly effect on the hemodynamic during laparoscopic ectopic pregnancy operation perioperative,the patients feel comfortable.The LMA is more suitable for short laparoscopy ectopic pregnancy operation.
