自身免疫性胰腺炎多种影像学比较分析

目的 探讨多种影像学检查对自身免疫性胰腺炎(AIP)的诊断价值.方法 回顾性分析我院2008年4月至2011年12月17例AIP患者的临床及影像学资料,男性13例,女性4例,年龄48～68岁,17例均行CT平扫及增强扫描；12例行磁共振成像(MRI)及磁共振胰胆管造影(MRCP)检查,9例行经内镜逆行胰胆管造影(ERCP)检查且7例同时行胆总管支架置入术.结果 CT、MRI既可以显示胰腺形态改变；也可以发现胰腺周围结构改变,而MRCP和ERCP可显示胰胆管结构改变.结论 自身免疫性胰腺炎影像学表现具有一定的特征性,结合多种影像学检查早期诊断和治疗对预后有重要意义.

Multiple imaging comparative analysis of autoimmune pancreatitis

Objective To investigate the imaging features of autoimmune pancreatitis by multiple imaging. Methods The clinical and imaging dates of 17 patients with autoimmune pancreatitis（13 men and 4 women;age range,48 - 68 years） were analyzed retrospectively. All cases underwent plain and enhanced CT scans, 12 eases performed MRI and MRCP examination, 9 cases were examined by ERCP and 7 cases underwent the common bile duct stent implantation. Results A capsule-like low-density rim and delayed enhancement of enlarged pancreas on cross-sectional imaging. Narrowed of the main pancreatic duct on ERCP and MRCP. Extrapancreatic involvement including stenosis of bile duct, enlargement of the salivary glands ,retroperitoneal fibrosis and obliterative phlebitis of pancreatic and peripancreatic veins. Conclusion The imaging manifestations of the autoimmune pancreatitis had typical radiographic feature, combinated with multiple imaging ,the early diagnosis and treatment are important to prognosis.
