摘    要:目的 探讨超声检查早期肝脾挫裂伤的应用价值,以寻求早期诊断的条件.方法 经超声检查发现的肝脾破裂患者30例,其中肝挫裂伤4例,肝裂伤并肝包膜下积液3例,肝挫伤并血肿形成1例,肝包膜下积液1例,脾挫裂伤10例,脾挫裂伤并包膜下积液3例,脾挫裂伤并血肿2例,脾挫裂伤并左肾挫裂伤2例,肝脾挫裂伤4例,对其超声形态特征进行对比分析.结果 本研究中肝脾挫裂伤以高回声为主,其间可夹杂液暗区.形态呈片状或不规则,边界不清楚.并腹腔积液17例,彩色多普勒血流影像很少探及血流信号.结论 依据病灶的二维超声形态特征可提高肝脾破裂的早期阳性发现率.

Abstract:Objective To study the morphological characteristics of early contusion and laceration of liver and spleen in order to seek the conditions for early diagnosis.Methods The ultrasonic morphological characteristics in 30 patients with liver and spleen rupture found by ultrasonic testing were performed the comparative analysis,which including 4 cases of contusion and laceration of liver,3 cases of liver laceration with subcapsular effusion,1 case of liver contusion with hematoma,1 case of liver subcapsular effusion,10 cases of contusion and laceration of spleen,3 cases of contusion and laceration of spleen with subcapsular effusion,2 cases of contusion and laceration of spleen with hematoma,2 cases of contusion and laceration of spleen and left kidney and 4 cases of contusion and laceration of liver and spleen.Results Contusion and laceration of liver and spleen were dominated by high echo mixed with fluidity shady area.The shape was flake-shape or irregular and the boundary was unclear.And 17 cases were complicated by ascites,in which blood flow signals were seldom detected by color Doppler flow image(CDFI).Conclusion The early positive detection rate of liver and spleen rupture can be improved according to the two-dimensional ultrasound morphological characteristics of focus.
