带蒂肋间肌瓣移植在预防支气管胸膜瘘中的应用

目的探讨带蒂肋间肌瓣移植在预防肺癌全肺切除术后支气管胸膜瘘（BPF）的应用价值。方法回顾性分析78例全肺切除术（A组）；135例全肺切除术（B组），常规用带蒂肋间肌瓣移植覆盖加固支气管残端。统计分析比较两组患者术后BPF发生情况。结果术后发生BPFA组4例（5．1％），B组为0例（0％）；两组比较差异有统计学意义（P〈0．05）。结论在肺癌全肺切除术中，常规带蒂肋间肌瓣移植覆盖加固支气管残端是降低肺癌全肺切除术后BPF的有效方法之一，其操作简单，安全性好，值得推广应用。

Effect of Pedicaled Intercostal Muscle Flap Transplantation in Preventing Bronchopleural Fistulat after Pneumonectomy

Objective To explore the value of the pediealed intercostal muscle flap transplantation（PIMFF） in preventing the post-pneumoneetomy bronehopleural fistula（BPF）. Methods Retrospectively reviewed the clinical data of lung cancer patients with pneumoneetomy. 78 eases（ group A） were treated with pneumoneetomy. 135 cases（ group B） were treated with pneumoneetomy and the proximal bronehia were covered by the pedicaled intercostal muscle. The incidence of BPF in the two groups were analyzed statistically. Results There were 4 cases of BPF in group A and 0 ease in group B. There was significant difference in the incidence of BPF between the two groups （ P 〈 0.05 ）. Conclusion To cover the proximal hronehia with pediealed intercostal muscle is a practical, easy and safe method to lower the incidence of BPF in the pneumonectomy. It is simple and good for popularization. It is worthy to be generalized in clinic.
