脂蛋白相关磷脂酶 A2 与缺血性脑卒中相关性的 Meta 分析

摘    要:目的:用Meta分析的方法探讨脂蛋白相关磷脂酶A2(lipoprotein-associated phospholipase A2,Lp-PLA2)与缺血性脑卒中发生的相关性。方法:在线检索PubMed,Web of Science,Embase和Cochrane Library以及中国生物医学文献数据库、中国期刊全文数据库、重庆维普和万方数字化期刊全文数据库中关于Lp-PLA2与缺血性脑卒中相关性的病例对照研究或队列研究,采用Stata 12.0软件对入选研究进行Meta分析。结果:共有8篇文献46 034例符合纳入标准。Meta分析结果显示:Lp-PLA2水平与缺血性脑卒中发生的合并效应量的危险比(95%可信区间)[RR(95% CI)]为1.04(0.98~1.11),提示Lp-PLA2水平与缺血性卒中发生无相关性。Lp-PLA2 活性与缺血性脑卒中发生的相关性合并效应量RR(95% CI)为1.03(0.96~1.10),提示Lp-PLA2活性与缺血性卒中发生无相关性。结论:目前不能认为Lp-PLA2水平及Lp-PLA2活性可以预测缺血性脑卒中的发生,高Lp-PLA2水平及高Lp-PLA2活性是缺血性脑卒中发病的危险因素的结论有待进一步论证。

Abstract:Objective: To analyze the relationship between lipoprotein-associated phospholipase A2 (Lp-PLA2) and ischemic stroke.Methods: Corresponding data, with case-control studies or cohorts regarding Lp-PLA2 and ischemic stroke, were retrieved from PubMed, Web of Science, Embase, the Cochrane Library, Chinese Biomedical Literature (CMB), VIP, China National Knowledge Infrastructure (CNKI), and Wanfang Database. Meta-analysis was performed by using Stata12.0.Results: Eight studies including 46 034 participants met the inclusion criteria. Meta-analysisshowed that the combined effect of relative risk (RR) between Lp-PLA2 mass and ischemic strokewas [1.04 (0.98 to 1.11)] and that the combined effect of RR between Lp-PLA2 activity andischemic stroke was [1.03 (0.96 to 1.10)], suggesting that Lp-PLA2 mass and activity were notassociated with ischemic stroke.Conclusion: It cannot be considered that Lp-PLA2 mass and Lp-PLA2 activity was the risk factorfor ischemic stroke.
