CO2气腹对新生猪血液动力学及呼吸功能的影响

摘    要:目的:建立新生猪气腹模型,观察CO2气腹中新生猪血液动力学及呼吸功能指标的变化规律。方法:新生猪共7头(7~14d,体质量2~4kg)。麻醉后监测心率(HR)、平均动脉压(MAP)、中心静脉压(CVP)、心输出量(CO)、每搏输出量(SV)、全身血管阻力(SVR)、动脉血气(ABG)。监测时间为麻醉稳定后气腹开始前5min(T0),气腹开始后15min(T1)、60min(T2)、120min(T3),气腹结束后30min(T4)。结果:新生猪CO2气腹后CO、SV下降(P〈0.05);MAP、SVR升高(P〈0.05);HR、CVP无明显变化;PaCO2增高(P〈0.05)。伴有血pH下降,表现为呼吸性酸中毒。CO2气腹不引起新生猪动脉血氧分压下降。结论:CO2气腹使新生猪血液动力学发生改变,通气功能下降;健康新生猪在严密监测下可以耐受短时间气腹。

Abstract:Objective: To observe the effects on the hemodynamic and ventilatory function in carbon dioxide pneumoperitoneum animal models. Methods:Seven piglets(2-4 kg,7 -14 days of age). They were put under general anesthesia to allow measurement of heart rate(HR), mean blood pressure(MAP), central venous pressure(CVP), cardiac output(CO), stroke volume(SV), systemic vascular resistance (SVR) and arterial blood gases(ABG). Time was recorded at intervals of 5 min(T0 ) before carbon dioxide pneumoperitoneum. 15min(T1 ) 60 min(T2 ) and 120 min (T3) after carbon dioxide pneumoperitoneum and 30 min(T4 ) after deflation. Results: The levels of SV and CO decreased signifcanly (P〈0.05), while the levels of MAP and SVR increased sig nifcanly (P〈0.05). There was no statistically signifcant alteration in HR or CVP. The pH decreased and PaCO2 increased by insufflation of carbon dioxide intraperitoneally with respiratory acidosis. There was no statistically signifcant alterations in PaO2. Conclusion: Carbon dioxide pneurnoperitoneum changed the hemodynamics and decreased the ventilate function in the piglets. The results of this study suggest that healthy piglets can tolerate CO2 insufflation well under general anaesthesia by monitor rigorously.
