病毒性心肌炎患儿脂质过氧化改变及抗氧化药物作用的研究

摘    要:对62例病毒性心肌炎(VMC)患儿进行血清丙二醛(MDA)、超氧化物歧化酶(SOD)及肌酸激酶心肌同功酶(CK-MB)检测,并以超声心动图测定其射血分数(EF)。结果示VMC患儿发病1周内MDA升高,SOD下降,MDA与CK-MB呈正相关,与EF呈负相关;SOD与CK-MB呈负相关,与EF呈正相关。经大剂量维生素C治疗后,MDA和SOD于4-6周恢复正常。体外实验发现,维生素C可增强过氧化氢(H2O2)损伤心肌细胞的活力,减少乳酸脱氢酶(LDH)释放和MDA产生,维持SOD水平;参麦注射液(SMI)的作用优于维生素C。认为测定脂质过氧化水平有助于VMC患儿的病情评价,SMI和维生素C对氧自由基损伤心肌细胞有保护作用,SMI疗效优于维生素C。

Abstract:We determined MDA and SOD in 62 children with viral myocarditis(VMC),and measured CKMB and ejection fraction (EF)at the same time,found that MDA increased and SOD decreased in VMC; MDA negatively correlated with EF and positively correlated CK-MB,SOD positively correlated with EF and negatively correlaated with CK-MB. After being treated with high dose of VitC,MDA and SOD returned to normal level in 4-6weeks. In vitro test, VitC could increase the viability of H2O2-injured cardiomyocytes,decrease LDH and MDA and keep SOD level in the culture media. Shenmai injection (SMI)could exert a better effect compared with VitC. This suggested that the level of lipid peroxidation could reflect seventy of VMC, SMI and VitC could protect cardiomyocytes from damage induced by oxygen free radicals.
