基底动脉尖综合征临床特点研究

目的探讨基底动脉尖综合征（TOBS）的临床表现、影像学特征、治疗及预后转归。方法回顾性分析40例TOBS患者的临床资料。结果 TOBS的临床表现多样,以眩晕、眼动及瞳孔变化多见,伴或不伴有意识障碍、偏盲、偏瘫等症状。部分患者有进展性加重,治疗以抗凝抗栓为主,预后差,病死率高。影像学特征：病灶分布于后循环支配区域,多见于丘脑、小脑、枕叶、中脑、聂叶底面。结论诊断TOBS应结合临床和影像,预后转归取决于发病后的就诊时间及病灶范围,超早期溶栓治疗应考虑为首选方案。

Clinical Andysis of 44 Patiens With Top of the Basilar Atery Syndrome

Objective To study the clinical manifestation,iconography character,therapy andprognosis of top of the basilar syndrome(TOBS).Methods To retrospective analysis the clinical data of 44 patients with TOBS.Results The clinical manifetation of patients with TOBS is vary,and circumgyration,eye movement and change of pupil of the eye are main manifestations in clinical,with or without the symptom disorader of consciousness,half-blindness,hemiparalysis,and so on.The sympotom of part of these patients is advancing aggravated,and the treatment to them is anticoagulation and antithrombus,the prognosis of them is bad and the case fatality rate is high.The iconography character:The nidus is in the region charged by posterior cerebral arterial circle,mainly in cerebral ganglia,cerebellum,loboccipitalis,midbrain and underside of temporosphenoid lobe.Conclusion The diagnous of TOBS should combine with clinicle and iconography,the prognosis of TOBS is based on visit time of the disease and the range of nidus.And the therapy of thrombolysis in super-early phase shoud be considered as the first therapy choice.
