胰岛素泵强化治疗过程中的护理问题及对策

目的探讨胰岛素泵强化治疗糖尿病（Diabetes mellitus DM）过程中可能出现的各种护理问题及对策。方法对24例使用由北京科联升华应用技术研究所提供的由韩国MESMED系统有限公司生产的胰岛素泵强化治疗的糖尿病（DM）患者进行心理护理和糖尿病教育并对患者情况进行系统性的回顾分析。结果24例糖尿病患者在治疗过程中出现的护理问题：输注障碍报警3例；胶贴过敏局部皮肤发痒2例；出汗多针头移位1例；低血糖2例；高血糖3例。采取适当的护理措施治疗5～12d后，血糖控制理想的患者23例，血糖控制较好的患者1例。结论熟练掌握胰岛素泵的操作程序，预见胰岛素泵可能出现的护理问题，给予适当的防范措施是胰岛素泵治疗顺利进行的有效保证。

Problems of Nursing Care and Countermeasure Insulin Intensive Treatment by Using Insulin Pump

Objective To investigate the possible problems in the insulin intensive treatment with insulin pump for patients with diabetes（ Diabetes mellitus,DM） and the nursing countermeasures. Methods 24 cases with DM received insulin intensive treatment by using insulin pump produces by the South Korean MESMED System Company,and the psychological nursing and educations on diabetes were simultaneously carried out. The clinical materials were analyzed retrospectively. Results The main problems of nursing care in 24 diabetic patients received insulin pump therapy were：（1）infusion obstacle alarm in 3 cases;（2）stickers allergic local skin itching in 2 cases;（3）sweat needle shift in 1 case. （4）low blood sugar in 2 cases;（4）high blood sugar in 3 cases. Appropriate nursing interventions of 5 to 12 days were taken, and the blood sugar got ideal controlled in 23 cases, well control controlled in 1 case. Conclusion In the insulin intensive treatment with insulin pump, nurses should clearly understand the operating rules of the insulin pumps, predict the possible caring problems and take appropriate preventive measures, and ensure the effective treatment of DM.
