前路手术治疗外伤性颈椎间盘突出伴脊髓损伤疗效分析

目的评价前路植骨钢板内固定治疗外伤性颈椎间盘突出伴脊髓损伤的效果，为相关研究提供参考。方法回顾性分析31例外伤性颈椎间盘突出伴脊髓损伤患者的临床资料，术前采用Frankel功能分级（A级4例，B级17例，C级7例，D级2例，E级1例），均采用前路手术减压、自体髂骨植骨及颈椎前路锁定钢板内固定术治疗，观察疗效。结果术后除2例Frankel分级A级和1例Frankel分级B级患者无恢复外，其余患者恢复1～2个等级，随访6～24个月，平均14个月，无钢板松动、断裂、植骨不融合等并发症。结论应用前路植骨钢板内固定治疗外伤性颈椎间盘突出伴脊髓损伤，可使受压脊髓充分减压，脊柱恢复稳定，损伤脊髓获得较好的恢复。

Surgical Treatment of Traumatic Anterior Cervical Disc Herniation with Spinal Cord Injury

Objective To analyze the clinical results of anterior interbody fusion with locking plate fixation for the treatment of traumatic cervical disc herniation with spinal cord injury. Methods The clinical data of 31 patients who suffered from traumatic cervical disc herniation with spinal cord injury were retrospectively reviewed. Neurologic status of all the patients was graded on Frankel scale ,which included 4 cases grade A,17 cases grade B,7 cases grade C,2 cases grade D and l case grade E. Surgical procedures included discectomy, strut bone grafting within vertebral bodies and internal fixation with locking plate. Results Of 28 patients who had the different degree recovery of nerve function. The postoperative foliow-up time was 14 months on average （ range 6 to 24 months）. There was no fusion failuer and complications of internal fixation. Conclusion Surgical procedures in- cluded discectomy, strut bone grafting with vertebral bodier and internal fixation with locking plate through anterior approach are helpful to recovery the patients＇ s nerve function who suffered from traumatic cercical disc herniation with spinal cord injury.
