经皮附睾和睾丸穿刺精子抽吸术在无精子症鉴别诊断中的应用

摘    要:目的 研究经皮附睾穿刺精子抽吸术(PESA)和睾丸穿刺精子抽吸术(TESA)在梗阻性与非梗阻性无精子症鉴别诊断中的应用价值.方法经3次或3次以上精液离心未发现精子而诊断为无精子症的149例患者,经睾丸体积测定、血清性激素水平和精浆生化指标检测、生殖系统B超等术前检查后,于局麻下行PESA或PESA+TESA(PESA/TESA),对抽吸获得组织进行组织病理学检查和精子计数分析;对其中睾丸体积和血清促卵泡激素(FSH)水平正常者加行睾丸切开活检,进一步评价生精功能.结果 PESA/TESA结果显示,149例中梗阻性无精子症82例(55%),非梗阻性无精子症67例(45%).PESA/TESA联合睾丸体积、血清FSH水平和精浆果糖等指标,对梗阻性与非梗阻性无精子症进行鉴别诊断的符合率分别为98.2%和78.8%.35例患者行睾丸切开活检,病理学检查结果与PESA/TESA结果一致.结论 PESA/TESA操作简单且创伤小,是对梗阻性与非梗阻性无精子症进行鉴别诊断的可靠方法.
