CT,IADSA对胰头癌诊断的比较研究

本文分析了65例术前CT诊断为胰头癌的CT征象和其中18例的LADSA表现,手术、病理证实CT诊断正确率为91％,提出胰头肿块是CT诊断胰头癌的直接征象,但并非特异性表现,需与胰头炎性肿块鉴别;“双管征”及胰周大血管脂肪层消失可出现在肿瘤,炎症等病变中。通过比较分析12例胰头癌和CT误诊的5例胰头炎性肿块的LADSA表现,作者认为通过显示胰头癌的肿瘤区异常血管,不规则、不均匀的“肿瘤染色”,胰周血管破坏,对胰头癌的诊断和鉴别诊断具有重要意义。

A Compartive Study of CT and IADSA in Carcinoma in Head of Pancreas

65 cases to be diagnosed as carcinoma in pancreatic head (CPH) by CT before operation were analysed, among them IADSA was done in 18 cases. The accurate diagnostic rate proved by pathology was 91%. The results showed that the mass of pancreatic head was direct finding of carcinoma, but it was nonspecific which should be differentiated from chronic pacreatitis: the "biductal sign", and loss of normal perivascular fat might be caused by either malignant neoplasm in pancraetic head or chronic pancraetitis. By comparitive analysis of the IADSA of CPH in 12 cases and inflammatory mass in pancreatic head in 5 cases which were misdiagnosed by CT, the authors suggested that irregular and in harmonous "tumor stain", anomolous vascularity in mass and involvement of peri pancreatic vascularity were significant in diagnosis CPH and differentiating diagnosis of CPH.
