副神经节瘤临床特征及诊治

目的:探讨副神经节瘤临床特点及诊断治疗。方法:回顾性分析2000年1月～2009年12月收治18例副神经节瘤患者的临床资料。结果:18例患者血压为150～230/90～150mmHg(1mmHg=0.133kPa),腹部疼痛4例,位于肾门旁8例,肾下极3例,腹主动脉前肠系膜根部3例,膀胱内2例,髂总动脉旁2例。手术治疗17例,良性15例,恶性2例。肿瘤直径3～11cm;1例在等待手术过程中并发脑血管意外死亡。15例随访1～10年,血压术后均有不同程度下降,其中12例于1周～0.5年完全恢复正常,3例服用降压药物后容易下降到正常范围。结论:副神经节瘤手术前定位困难,容易漏诊,手术治疗是最有效的方法,术中血压波动较大,需要作充分手术前准备。

The dagnosis,treatment and clinical characters of paragangliomas

Objective:To investigate the clinical characters,diagnosis and treatment of paragang-liomas.Methods:The clinical documents of 18 cases paragangliomas were retrospectively reviewed.Results:Hypertensions was observed in all 18 cases.Intermintent Abdominal pain was seen in 4 cases.8 cases located at renal hilum,3 cases at the lower pole of kindey,2 cases at the anterior of abdominal arota,2 cases in the bladder wall,2 cases at the anterior of common iliac artery.15 cases were benignand and 2 cases were malignant.1 case died of cerebral hemorrhage before operation.15 cases were followed up 1-10 years,the blood pressure of 15 cases reduced,in 12 cases the blood pressure returned to normal 1 week to half a year after operation.2 cases the blood pressure can easily be controled to normal by take nifedipine or captopril.Conclusions:Detection of paragangliomas before operation is difficult,surgial resection is the best way in the treatment of paragangliomas.
